Using data from infant observation, and child, adolescent, and adult analyses, the Novicks explicate a multidimensional, developmental theory of sadomasochism that has been recognized as a major innovation. According to the Novicks, each phase of development contributes to the clinical manifestations of sadomasochism. Painful experiences in infancy are transformed into a mode of attachment, then into an embraced marker of specialness and unlimited destructive power, then into a conviction of equality with oedipal parents, and, finally, into an omnipotent capacity to gratify infantile wishes through the coercion of others. By school age, these children have established a magic omnipotent system of thought which undermines alternate means of competent interactions with reality. In adolescence and adulthood it becomes increasingly hard for them to deny, avoid, or distort reality without resorting to escalating self-destructive behaviors. Sadomasochistic phenomena are the source of severe resistances and counterreactions in all phases of therapy. This book helps clinicians recognize and overcome these blocks to treatment progress and success. Here can be found an introduction to the Novicks' reformulation of the therapeutic alliance, and their distinctive contributions to the transformations of memory and the termination of treatment.

Download Aggression in Personality Disorders and Perversions Book in PDF, ePub and Kindle
In this important new book, Dr. Otto F. Kernberg, one of the world's foremost psychoanalysts, explores the role of aggression in severe personality disorders and in normal and perverse sexuality, integrating new developments in psychoanalytic theory with findings from clinical work with severely regressed patients. The book also integrates Dr. Kernberg's recent studies of the descriptive, structural, and psychodynamic features of problems stemming from pathological aggression with the vicissitudes of their psychoanalytic treatment. Finally, Dr. Kernberg demonstrates the importance of differential diagnosis for effective psychoanalytically inspired treatment of these disorders, providing a rich variety of clinical illustrations. The book begins by relating the dual-drive theory of libido and aggression to contemporary developments in affect theory. Dr. Kernberg then applies this general theory of affects to aggression, which in its pathological form centers on the affect of hatred. He analyzes sado-masochistic, hysterical-hysteroid, and narcissistic-antisocial spectrums of personality disorders, emphasizing how aggression is structured in each group. Dr. Kernberg next describes and updates the theoretical frame underlying his approach to the treatment of these disorders, outlines their clinical manifestations, and illustrates their diagnosis and treatment, ranging from standard psychoanalysis with infantile personalities, to psychoanalytic psychotherapy with borderline personalities, to the psychotherapeutic approach to the treatment of psychosis and hospital milieu treatment in the management of highly regressed patients. In the final section, Dr. Kernberg links the findings from psychoanalytic approaches to personality disorders with those from the psychoanalytic study of sexual perversions.

Addressing the art and science of psychodynamic treatment, Nancy McWilliams distills the essential principles of clinical practice, including effective listening and talking; transference and countertransference; emotional safety; and an empathic, attuned attitude toward the patient. The book describes the values, assumptions, and clinical and research findings that guide the psychoanalytic enterprise, and shows how to integrate elements of other theoretical perspectives. It discusses the phases of treatment and covers such neglected topics as educating the client about the therapeutic process, handling complex challenges to boundaries, and attending to self-care. Presenting complex information in personal, nontechnical language enriched by in-depth clinical vignettes, this is an essential psychoanalytic work and training text for therapists.

Download The Paraphilias Book in PDF, ePub and Kindle
Of the thousands of papers and books about problematic sexual behaviors, most focus solely on sex crimes or so-called "hyper-sexuality" or "sexual addiction." Together, these publications present a grim and pessimistic prognosis for anyone who has unusual sexual interests of any type. This book challenges that view by providing a more informed and balanced review of what is known and what is not known about unconventional sexual interests. It is based on approximately thirty years of experience by the author concerning the assessment and treatment of paraphilias and unconventional sexual interests. The Paraphilias: Changing Suits in the Evolution of Sexual Interest Paradigms examines current and past perspectives concerning unconventional sexual interests associated with both criminal and non-criminal activities. Extensively referenced, it challenges the dogma that sexual interests are immutably determined during a single critical period and are thereafter unchangeable. The book provides extensive case histories and tables summarizing over 100 paraphilias and the latest research regarding them. It also reviews diagnostic criteria for the paraphilias. Analyses of current and past paradigms are presented together with new ways to understand, investigate, and provide meaningful and effective assistance to people with paraphilias. It is written for mental health clinicians and specialists in the fields of sexology and forensic psychiatry and psychology.
