Abstract The purpose of this Clinical Nurse Leader (CNL) project was to reduce gaps in nursing knowledge through staff education. Guided by the CNL theme of educator and information manager, the goals were to increase PreAdmission Testing (PAT) nurses' knowledge of severe/critical aortic stenosis (AS), transcatheter aortic valve replacement (TAVR) procedure and patient selection, and satisfaction with knowledge by 10%. Rapid growth of the TAVR program has contributed to a gap in nursing knowledge leading to inconsistent patient care and education. An educational program was developed and spread through a series of Plan-Do-Study-Act cycles. Pre- and post- implementation surveys were conducted. Instructional methods included brief individual training sessions, use of a TAVR video, educational packets, wall posters, and a formal one-hour inservice. Results show an average improvement of 26%-52% of "good" to "excellent" responses to survey questions. Project goals were exceeded in each area. Recommendations include further expansion of educational program to other units providing care for TAVR patients.

Transcatheter aortic valve replacement (TAVR), sometimes called transcatheter aortic valve implantation (TAVI), was developed as an alternative for patients with severe aortic stenosis, who require aortic valve replacement but who are not eligible for conventional surgical aortic valve replacement (SAVR). According to a 2011 report, approximately 300,000 people worldwide have been diagnosed with severe aortic stenosis and approximately one-third of them are considered too high risk for open heart surgery. Currently, the two most common approaches for TAVR are transfemoral and transapical procedures. There are two commercially available systems for TAVR: Edwards Sapien (LifeSciences, Irvine, CA, US) and CoreValve (Medtronic, Minneapolis, MN, US). The benefits of TAVR with up to one year follow-up were demonstrated in two randomized controlled trials (RCTs) -- PARTNER cohorts A and B -- which showed that TAVR has statistically significant clinical benefits compared with standard therapy or SAVR, as summarized in a previous Rapid Response review. With the aim to review long-term success and complication rates of the procedure, this report will provide a review of the use of TAVR at more than 12 months follow-up in patients with severe aortic stenosis, compared with SAVR or standard treatment (medical therapy plus balloon aortic valvuloplasty if needed).

Aortic Valve Transcatheter Intervention Calcific aortic stenosis (AS) is the most common heart valve anomaly, with a largely age-dependent prevalence, a calculated annual incidence rate in the range of 4-5% in general populations and up to 6% in patients aged 75 years and over. Surgical aortic valve replacement (SAVR) was previously the only option available to patients with symptomatic, severe aortic stenosis. After the first-in-human transcatheter aortic valve implantation (TAVI) was performed by Alain Cribier in 2002, the treatment strategy for patients with symptomatic AS has been revolutionized. Since then, TAVI has grown exponentially, as a result of accruing evidence demonstrating safety and efficacy, and reduced invasiveness compared with SAVR. TAVI devices are continuously expanding to include several valve design options. As this strategy is continuously evolving to treat younger patients and lower-risk populations, aside from the long-term durability of the valve systems, procedural safety will become the focus of newer-generation devices.
