The objective of the program is to establish at the University of Texas Health Science Center at San Antonio an in-depth training program in the Molecular Genetics of Breast Cancer. One goal of the program is to train highly qualified doctoral students in the genetic, cellular, and molecular basis of Breast Cancer. The training program, conducted within the Molecular Medicine Ph. D. Program, is administered by a select group of faculty whose research projects are intimately involved in breast cancer. An additional goal of the program is to promote synergistic interactions between the various laboratories engaged in breast cancer research. Breast cancer meetings, Molecular Medicine Minisymposia and a distinguished Seminar series are integral parts of the training program. The strength of the program is the quality of the Program faculty, and the interactive breast cancer research community. The faculty are studying breast cancers and their therapy, as well as fundamental mechanisms of cell growth and differentiation. Students supported by the training program had 22 peer-reviewed publications, a 400% increase over 1996. Based on the excellent progress of the training program, a proposal to the National Institutes of Health for continuation of the Program will be submitted June, 1998.

The three campuses of the of the Mayo Graduate School provide an advanced clinical and research training environment for conducting an integrated. multidisciplinary predoctoral training program based in the study of cell and molecular biology of breast cancer. Thirty-two full-time faculty members currently participate in Mayo's didactic tumor biology training curriculum which supports the training of postdoctoral trainees in NCI sponsored postdoctoral training programs at Mayo. This strong multidisciplinary tumor biology curriculum provides the foundation for the didactic component of a specialized predoctoral training program in breast cancer. The development of this new training program has been facilitated through the development of two new graduate courses in the cell and molecular biology of breast cancer, as well as through the initiation of a new journal club and an intramural research workshop for trainees in this training program. The research training component of this new predoctoral track is laboratory-based and is fostered by Mayo' highly competitive and interdisciplinary research environment and by the heightened interest in breast cancer research stimulated through the recent development of the Mayo Women's Cancer Program within the Mayo Cancer Center.

The goal of this research training program is to produce highly qualified scientists for careers as independent investigators in the field of breast cancer. During the last 25 years, there has been a fundamental revolution in the understanding of molecular and cell biological concepts related to cell growth, function and tumorigenesis. To utilize what has been learned and to continue future progress in the area of breast cancer requires the continued availability of well-trained, innovative and committed scientists. This program represents an interdepartmental training program involving 15 investigators from seven departments. Trainees are predoctoral and postdoctoral fellows with backgrounds in bio- chemistry, cell and molecular biology, molecular genetics and molecular virology. The training program provided trainees with additional foundation in carcinogenesis and breast cancer. In addition to the core curriculum taken by the predoctoral fellow in their respective academic departments, program enhancement is provided through trainees' participation in a graduate course on "Molecular Carcinogenesis" (predoctoral fellows), a Breast Disease Research Seminar (all trainees) and participation at national meetings and local seminars. Three predoctoral and three postdoctoral trainees are enrolled in the program.

Breast cancer is one of the leading causes of cancer deaths of women in the United States. Fortunately, this disease is no longer a "black box" that can only be studied empirically. Recent advances in understanding of normal mammary development and carcinogenic processes have identified a number of specific genes and processes that are dysregulated in breast cancer. This means that research on breast cancer has finally advanced to the stage where a concentrated effort in translational research will yield great strides in detection, diagnosis, and treatment. The Molecular Medicine graduate training program at Yale was recently developed to address these issues. This program was developed to offer an interdisciplinary course of study that will foster an integrated view of disease, built upon a rigorous foundation of basic sciences. The emphasis on disease mechanisms and translational research is unique to Molecular Medicine, and distinguishes it from other pre-doctoral programs at Yale. The Predoctoral Training Program in Breast Cancer Research will recruit individuals interested in careers in breast cancer research to the Molecular Medicine Program, provide specialist training in breast cancer-specific areas, and integrate their training experience with scientists and clinicians investigating breast cancer at Yale.

With the continuous funding from the US army/DOD, the training program on breast cancer research at MDACC has had a successful third year. The training program has supported four predoctoral and two postdoctoral fellows. Each trainee has made notable progress as evidenced by publications and presentations at national meetings. Significant strides have been made within the scope of the original specific aims in the following research areas: 1) Therapeutic approaches for breast cancer through regulation of oncogene and tumor suppressor gene expression, and control of signal transduction, apoptosis, and DNA repair; 2) Use of animals to understand the biology of breast cancer and to provide models for preclinical therapeutic and preventive studies; 3) Novel preventive strategies for breast cancer; 4) Population-based studies on breast cancer; 5) Molecular diagnostic/prognostic factors for breast cancer; and 6) The basis biology of breast cancer. In addition to laboratory pursuits each trainee has participated in departmental group meetings, journal clubs, and retreats. The goal of the training program is to further the successful training of fellows who will develop research programs of their own which continue to tackle problems of breast cancer.

Objective: The overarching goals of this partnership between Charles F. Drew University of Medicine and Science (Drew) and the University of California Los Angeles Jonsson Comprehensive Cancer Center (UCLA-JCCC) are to develop Drew faculty into independent and competitive investigators and in turn, provide Drew with the necessary capacity to establish a breast cancer research program with a focus on minority and underserved populations. We have generated significant progress toward the following objectives: 1) We have created a Cancer Cluster at Drew that brings together various investigators engaged or have interest in Cancer Research, Education, Training, and Treatment. This Cluster group has enabled us to identify senior, mid-level, and junior faculty who can be partnered with established breast cancer investigators at UCLA as senior mentors or research collaborators. 2) We have created an educational experience and mentored research environment that fosters breast cancer research and offers substantive training to Drew (HBCU/MI) faculty and postdoctoral fellows. 3) We have created opportunities for Drew faculty, fellows and students to access UCLA-JCCC and Drew shared resources. 4) We have generated exciting data that identifies novel genes that may be associated with HER2/neu overexpressing breast tumors. In particular, candidate genes that may be associated with resistant to Trastuzumab treatment.

The Northwestern University Robert H. Lurie Comprehensive Cancer Center is an NCI-funded comprehensive cancer center. One of the major accomplishments for the Cancer Center has been to establish a nationally recognized program in both laboratory and clinical based breast cancer research. The Cancer Center has received $14.2 million from the Avon Foundation and successfully competed for a SPORE in Breast Cancer from the National Cancer Institute. In September 2000 the Cancer Center received a four year award from the US Army for comprehensive training of graduate students and postdoctoral fellows conducting breast cancer relevant research entitled, "The Molecular Biology of Breast Neoplasia" (DAMDl7-00- 0386). In year 2003, the Selection Committee reviewed applications and assisted in the selection of 4 graduate students and two postdoctoral fellows for appointment to the Training Program. Trainees and mentors actively participated in the weekly meetings of the Breast Cancer Journal Club, as well as training program course entitled, Advanced Topics in Breast Cancer. Year 2004 appointments have just been completed.

X-ray mammography screening is the current mainstay for early breast cancer detection. It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.
