This handbook provides an overview of thyroid nodules, their testing, and typical follow-up options. As this handbook notes, most thyroid nodules are benign, not cancerous. Please note: The information in this handbook is intended for educational purposes and is for general orientation. This atlas is the offspring of the “The National Cancer Institute (NCI) Thyroid Fine Needle Aspiration (FNA) State of the Science Conference,” hosted by the NCI and organized by Dr. Andrea Abati. Preparations for the conference began 18 months earlier with the designation of a steering committee and the establishment of a dedicated, p- manent web site. The meeting took place on October 22 and 23, 2007 in Bethesda, Maryland and was co-moderated by Susan J. Mandel and Edmund S. Cibas. The discussions and conclusions regarding terminology and morphologic criteria 1, 2 from the meeting were summarized in publications by Baloch et al. and form the framework for this atlas. The atlas is organized by the general categories of “Nondiagnostic,” “Benign,” “Follicular Neoplasm/Suspicious for a Follicular Neoplasm”, “Suspicious for Malignancy,” and “Malignant,” and it includes the defi- tions and morphologic criteria of these categories as set forth by Baloch et al. The majority of the conference participants also agreed on a category of “undetermined significance,” which is incorporated in this atlas (Chap. 4). It is critical that the cytopathologist communicate thyroid FNA interpretations to the referring physician in terms that are succinct, unambiguous, and helpful clinically. We recognize that the terminology used here is a flexible framework that can be modified by individual laboratories to meet the needs of their providers and the patients they serve.

Utilizing the most current evidence, this practical book is the first publication of its kind to focus exclusively on the clinical and laboratory evaluation and therapy of thyroid nodules, including advances in evaluation and controversies in management. Opening with chapters presenting the epidemiology, history, laboratory and clinical evaluation of thyroid nodules, the use of radioactive isotope ultrasound and CT scanning in diagnosis is then described. The interpretation of thyroid cytology following fine needle aspiration (FNA) biopsies and the use of molecular markers are discussed in detail, as is the controversial use of thyroxine therapy. Following a presentation of the common and minimally invasive surgical approaches to thyroid nodules, chapters on multinodal goiters, incidentalomas and thyroid nodules in children round out the presentation. Written and edited by leading US and international thyroidologists, Thyroid Nodules is a useful, state-of-the-art text for endocrinologists, surgeons, and primary care physicians.

Thyroid Disease: A Case-Based and Practical Guide for Primary Care, by Dr. Emanuel Brams, is a supremely readable cased-based approach to understanding the myriad of disorders that encompass thyroid disease. Using cases derived from his busy practice, Dr. Brams gives us the history and physical examination details of each case, along with test results. Then, through a series of questions similar to that which any good clinician asks while caring for a patient, Dr. Brams explains an optimal approach to diagnosis, treatment, and follow-up of thyroid disease, with an emphasis on the practical and common issues that primary care physicians see in their offices. This is an important book for primary care physicians because thyroid disease is common in the population we take care of and accounts for 3–5% of primary care visits. It is often cited as an area that primary care phy- cians find confusing. Dr. Brams clarifies thyroid disease and, through his generous sharing of cases, allows us to join him in making clear diagnoses as well as share in the ambiguity and decisions about approach when the diagnoses and course of treatment are not clear. In this way, the book is honest, and deals directly with the issues of thyroid disease as they are seen by us as physicians, not as we sometimes may wish they were seen if patients presented with the clarity of walking textbooks. Dr.

This book provides a practical, evidence-based approach to controversies in the management of thyroid nodules and differentiated thyroid cancer (DTC). Thyroid nodules are a commonly encountered clinical entity, identified in 2-6% of the population by palpation, and 20-35% by ultrasound. DTC is the solid organ cancer with the most rapidly increasing incidence (>50,000 new cases in the U.S. annually). Despite extensive research and multiple published comprehensive practice guidelines, numerous controversies remain within this field covering a wide range of issues, including incidence and epidemiology, radiographic evaluation, criteria for needle biopsy, cytopathologic and molecular diagnostics, active surveillance vs. operation for low-risk tumors, extent of surgery, indications for radioactive iodine treatment and external beam radiation, and optimal protocols for follow-up. The book takes a case-based approach: each chapter will begin with a patient presentation highlighting a particular controversial subject in thyroid nodules and DTC. The remainder of the chapter discusses the controversy within the context of current scientific understanding and clinical practice, using the latest evidence-based practice guidelines. Chapters conclude with a final review and discussion of the opening case, with options and recommendations for next steps provided. The recommendations are evidence-based and practical, intended to help guide practitioners in their ongoing care of patients with thyroid nodules and DTC. Each chapter was written by a recognized expert in the area of controversy, including endocrine surgeons, endocrinologists, radiologists, medical oncologists, and nuclear medicine specialists. Controversies in Thyroid Nodules and Differentiated Thyroid Cancer: A Case-Based Approach is an excellent guide for oncologists, endocrinologists, otorhinolaryngologists, surgeons, and practitioners of all types that care for patients with thyroid nodules and DTC.
