Current cancer treatments, including surgery, radiotherapy and chemotherapy, have shown great success in treating solid and early metastatic tumors. Recently, investigators have examined the effects of these cancer treatment in promoting cancer metastasis. Based on recent studies, experimental evidence suggests that surgery, radiotherapy and chemotherapy induce an influx of circulating tumor cell (CTCs) into the bloodstream due to the disruption and destabilization of the tumor microenvironment. In a natural process of cancer metastasis, which is the main cause of cancer-related death, CTCs are shredded from the primary tumor into the bloodstream then seed and form macrometastases in distant organs. Once CTCs enter the blood, they face several stresses that can destroy them during transitional migration. Thereby, CTCs may travel in clusters with stromal cells, such as cancer-associated fibroblasts (CAFs), to enhance their survival in the bloodstream and improve the overgrowth of these cells in distant tissues. The scope of the work presented is divided in four sections, including: (1) A determination of the fluctuation of CTCs and CAF numbers, and the efficacy of a CTC-targeted therapy in blood samples collected from localized cancer disease undergoing prostatectomy, (2) A evaluation of the fluctuation of CTC and CAF numbers, and the efficacy of our previous mentioned therapy in blood samples collected from metastatic cancer disease undergoing chemotherapy treatment chemotherapy, (3) An investigation of the function of CAF incorporated into CTC clusters as a collective migration unit in prostate cancer progression, and (4) The development of a novel platelet-based therapy to kill CTCs with the goal of preventing metastasis.

This book is aimed to summarise the key aspects of the role of circulating tumour cells (CTCs) in breast cancer, with special attention to their contribution to tumour progression and establishment of metastatic disease. We aim to give a clear overview of the knowledge about CTCs, framed in the context of breast cancer, by analysing basic and clinical research carried out so far. In a broader sense, we will address what are the main clinical needs of this disease based on its molecular heterogeneity (subtypes) and lay out the knowledge and understanding that CTCs are giving about it and how they are contributing and can still improve the better monitoring and management of breast cancer patients. We will discuss the evidences of the use of CTCs as a tool to monitor cancer progression and therapy response, based on the prognostic and predictive value they have, as well as a tool to unravel mechanisms of resistance to therapy and to identify new biomarkers allowing to predict therapy success. Moreover, we will analyse the main aspects of ongoing clinical trials and how they can contribute to determine the clinical utility of CTCs as a breast cancer biomarker. We will also touch upon general knowledge or basic notions of the biology of the metastatic process in epithelial cancers, in order to understand the origin and biology of CTCs. In this sense, we will pay special attention to EMT (epithelial to mesenchymal transition), dormancy and minimal residual disease, three key aspects that determine the outcome of the disease. We will also cover general aspects on the isolation and characterization techniques applies to the study of CTCs, and also the possibilities that the study of CTCs, as a biomarker with biological function, is opening in terms of understanding the biology of metastatic cells and the identification of therapeutic targets based on the functional and molecular characterization of CTCs. Lastly, we will try to foresee the future of CTCs in terms of clinical application and implementation in the clinical routine.

It is well recognized that blood could be the optimal site for evaluating cancer, allowing easy and repeated access for determining prognosis, establishing molecular targets, evaluating the efficacy of therapy, detecting the earliest signs of recurrence, and even detecting cancer at its earliest and most curable stages. The analysis of cancer through blood samples is now known as the liquid biopsy and has been a rich source of research and clinical application. There has been an explosion of interest and progress in liquid biopsy technologies since the first edition of this book. The second edition will expand its focus to now include not only circulating tumor cells (CTC), but also other emerging aspects of the liquid biopsy, including circulating tumor DNA and methylated DNA (ctDNA, ct meDNA), ctRNA, ct miRNA, circulating tumor proteins (and other) biomarkers and circulating tumor derived exosomes (ctExosomes). Circulating tumor cells have been a major area of focus for cancer metastasis research for the last few decades, and while the majority of progress has mainly centered on the isolation and enumeration of CTC for prognostic purposes, researchers are beginning to explore additional uses for CTC in the clinical setting. While it is well known that CTC counts correlate with prognosis, CTC counts are also being explored as a marker for response to therapy. CTC enumerated before and after surgical procedures are shedding light on the advantages of minimally invasive techniques. CTC characterization has also increased in the past decade. When CTC were first reliably isolated, the majority of work focused solely on EpCAM-expressing cells found within the blood of patients with late stage cancer. Today, EpCAM-negative CTC and CTC expressing mesenchymal markers are under great scrutiny for a potentially increased metastatic potential. Moreover, CTC that differ in protein and gene expression to the primary tumor have been found elucidating prospective issues with drug targets. CTC have been isolated and identified in patients with local tumors or even pre-cancerous lesions. Here, we present a diagnostic method that uses CTC to prescreen for drug susceptibility. This method is then used to show primary CTC sensitivity to a novel TRAIL-based immunotherapeutic targeted towards CTC. Peripheral blood samples are shown to contain circulating stromal cells believed to migrate alongside CTC and likely alter their survival under shear conditions as well as their drug susceptibility. We culminate these studies by conducting the first efficacy study of EST liposomes in an immunocompetent breast cancer tumor resection murine model. ...
