Excerpt from The Surgical Treatment of Ulcer of the Stomach The reason of this discrepancy is simply that the condition of the mucous membrane of the stomach during life, especially when it is inflamed, is absolutely different from what it is after death. During life, and especially while digestion is going on, the mucous mem brane is exceedingly well supplied with blood. It is soft, bright red, thick, and full of fluid. After death it is thin, bloodless, flaccid, and in colour a pale slaty gray - it hardly looks like the same structure, the difference is so great; and in cases of acute ulceration, owing to the inflammatory changes in the surrounding tissues, the difference is greater still. Immediately around the ulcer the mucous membrane is swollen, con gested and raised above the level of the rest. Its colour is bright red, though the interior of the stomach may be paler than natural from loss of blood. Here and there I have seen the surface dotted with little blood-stained points from vessels that have given way. The sides of the ulcer are thickened and oedematous, and so soft and swollen that if the ulcerated surface is small, and does not extend deeper than the mucous membrane, they meet together over it and convert it into a narrow fissure. After death, when the blood pressure has fallen, especially in cases in which the vessels have been emptied by previous haemorrhage, all this thickening disappears, and then the ulcer assumes the crater-like form which is usually described; but I have never seen this during life. Excerpt from The Surgery of the Stomach: A Handbook of Diagnosis and Treatment IN the following pages I have attempted to give a practical account of The Diagnosis and Treatment of those Affections of the Stomach which are amenable to Direct Surgical Inter ference. 1 The significance of the information to be gained from test-meals is considered in some detail, because I believe that such investigations are of great value in the diagnosis of gastric disease. It appears to me strange, that whereas at the present time, bacteriological and pathological examinations are used so extensively in the diagnosis of disease, chemical methods are so rarely made use of in practice, and dealt with so briefly in text-books. The stomach-tube should be part of the armamentarium of every practitioner. It is not essential that he should perform all the quantitative analyses to which I refer, but I venture to think that he would be repaid amply, if he carried out the ordinary qualitative tests, and the estimation of the total acidity of the gastric contents. The practitioner, be he consultant or otherwise, who, without the aid which such investigations afford, continues to treat a patient suffer ing from persistent indigestion, or gastric pain, is acting unfairly to his patient and unjustly to himself. Excerpt from Section on Surgery: General and Abdominal Whether the case be a resection for cancer or a gastro-enterostomy for ulcer, the reserves of the starved patient must be built up by the introduction of water, sodium bicarbonate and glucose and especially by an adequate amount of sleep and rest; and acid formation must be diminished by the elimination, as far as possible, of worry, fear, anxiety, exertion, loss of Sleep, trauma and anesthesia. In the case of ulcer, the fate of the ulcer-bearing area is the chief consideration, for the evidence on many Sides, especially that presented by the Mayo Clinic, shows the tendency to cancer growth in ulcers of the stomach, though happily not in duodenal ulcers. Our lamented President, Dr. Rodman, made a logical departure in the complete excision of the ulcer-bearing area, but even this is not enough. At this point we must join hands with our medical confreres and for a period of many months - Six or more - the patient Should be kept under strict dietetic care, such, for example, as the Sippy regimen. If the patient is dis missed from the surgeon's care to the same habits of life, and the same dietetic carelessness that existed when the ulcer originated, and if in certain cases mouth infections and teeth defects are uncorrected, then there is bound to result a certain percentage of failures. It is just as illogical to exercise no care over a postoperative ulcer patient as it would be to dismiss a postoperative exophthalmic goiter patient without therapeutic instructions and prolonged oversight. Excerpt from Ulcer of the Stomach The ultimate object of our investigations into the causes of, and abnormal processes concerned in, a disease is the scientific treatment of our patients, the aim of which is the restoration to the normal of the morbid processes constituting the malady. In order to do this, it is essential to know what these processes are, and the relative part that each individually plays in the production of the clinical phenomena of the disease. The physician is unable effectively to study these processes of disease in man, because, although our methods of clinical research have vastly improved during the past few years, they are yet very limited, and particularly those dependent upon experiment and not upon mere observation. Again, when the case comes under observation, so many processes are usually disordered together that it is impossible to say which are of primary and which of secondary importance. It is, therefore, essential in order to study the causes of, effects of, and phenomena exhibited by isolated morbid processes at various stages in their course, to resort to experiments upon animals; but unless the anatomical basis of a disordered process in the human being has been previously ascertained, one has no idea how to reproduce the condition in an animal. The study of morbid anatomy and clinical medicine is thus inextricably blended with that of experimental pathology. It is true that certain pathological problems can be studied exclusively in the laboratory, but these are merely parts of the whole, and eventually must be correlated with the facts obtained through a study of the patient. The pathological and clinical aspects of a disease must, therefore, be studied side by side, otherwise our knowledge of it can never be precise and complete. Ten years ago ulceration of the duodenum was looked upon as a rare disease, and its confident recognition during life was believed to be hardly possible. To-day we know that this disease is common, and its discovery in the majority of cases presents no great difficulties to the trained clinician. In my own experience the diagnosis of duodenal ulcer is made with a degree of accuracy that is not exceeded in the case of any other abdominal disorder. Perhaps the most remarkable feature in the history of this condition is the fact that its main symptoms have through generations of text-books been ascribed without reserve or hesitation to the existence not of an organic but of a purely functional disturbance. The varied and accumulating experience of a few surgeons soon led, however, to the conviction that the range of functional disorders of the stomach must for the future be greatly narrowed, that not a few of the so-called "neuroses" of the stomach were veritable examples of structural changes having their seat in the stomach, duodenum, gall-bladder or appendix.
