Excerpt from Philadelphia Medical Times, Vol. 6: A Bi-Weekly Journal of Medical and Surgical Science; April 1, 1876 Ligature ofthe external iliac artery BY AN antiseptic material, for the cure or AN unusual number or aneurisms Oliver Pemberton, Ed, reports in the London Lame! Of February 26 the case of a patient 47 years of age, in whom were present in the same limb three considerable aneurismal tumors, - namely, one beneath Poupart's ligament, one at the apex of Scarpa's Space, and one in the popliteal region, - and in whom he tied the external iliac with a carbolized animal ligature. Three weeks after the operation the wound was soundly healed, having for the first fourteen days been treated thoroughly antiseptic, and for the subsequent ones dressed with a resin ointment to remedy some adjacent excoria tion. The aneurismal tumors at the time were firm and contracting in dimensions. Gangrene, which had occurred, ended by a well-defined line at the middle of the leg. The dead part being dry, absolutely without putre faction, under an antiseptic dressing of cotton wool prepared by saturation in a solution of carbolic acid in ether. The patient was well, and, beyond suffering pain near the line of demarcation, was undisturbed by the death of his foot. Salicin IN rheumatism. - Dr. T. Mac lagan has used salicin in rheumatism. He employed it in eight cases, and arrives at the following conclusions: I. We have in salicin a valuable remedy in the treatment of acute rheumatism. In four or five days she had entirely recovered, another examination showing that the wound had healed. In another instance a peasant pressed into the vagina of his wife, an idiot, the upper part of a wineglass, the stem of which was broken off. Twenty-four hours later, when she was first seen, the belly was much enlarged, her linen soaked with blood no urine had been passed for thirty-six hours. The glass was removed with the aid of a Sims speculum and a pair of forceps, but was broken during this operation, and the vagina was again lacerated. To settle this, a number of experiments were made by injecting putrid uids into the peritoneal cavities of rabbits. From these experiments he concludes that splenic enlargement is not always accompanied by the presence of micrococci, and that when they are found it is only during the first four or five days after infection. More of them were certainly too large to find their way so far as to the smaller vessels of the brain, and they were all unquestionably the result of re cent in ammation. Dr. Longstreth said he had met the state ment, but could not now recall its source, that emboli derived from vegetations on the mitral orifice were smaller than those coming from a similar condition of the aortic valve. The ex planation offered was that, having a greater distance to travel, and also from the fact of being within the cavity of the ventricle dur ing at least one contraction of the muscular wall of the heart, the emboli of mitral origin were more likely to be broken into very small fragments than those of aortic origin.
