A revolution has occurred during our lifetime in the collection, storage, and communication of information, a revolution whose full significance is scarcely understood even by those responsible for its development. At the core of this revolution, and its most visible component, is electronic data processing via the computer. The computer has shown an impressive ability to handle increasing complexity at greater speeds while decreasing its use of power, cost, and space. In this book, however, John McHale shows how the impact of the information revolution stretches far beyond these specific developments. It lies (1) in the expansion and interlinkage of computer-based systems and their extension into larger areas of automated control; (2) in the convergence and relationship of such systems with concurrently developing communications technologies; and (3) in the ways in which these combine together to create a radically new information environment, whose major impacts will be felt most critically within the next ten to twenty years. In 1996, the Institute of Medicine (IOM) released its report Telemedicine: A Guide to Assessing Telecommunications for Health Care. In that report, the IOM Committee on Evaluating Clinical Applications of Telemedicine found telemedicine is similar in most respects to other technologies for which better evidence of effectiveness is also being demanded. Telemedicine, however, has some special characteristics-shared with information technologies generally-that warrant particular notice from evaluators and decision makers. Since that time, attention to telehealth has continued to grow in both the public and private sectors. Peer-reviewed journals and professional societies are devoted to telehealth, the federal government provides grant funding to promote the use of telehealth, and the private technology industry continues to develop new applications for telehealth. However, barriers remain to the use of telehealth modalities, including issues related to reimbursement, licensure, workforce, and costs. Also, some areas of telehealth have developed a stronger evidence base than others. The Health Resources and Service Administration (HRSA) sponsored the IOM in holding a workshop in Washington, DC, on August 8-9 2012, to examine how the use of telehealth technology can fit into the U.S. health care system. HRSA asked the IOM to focus on the potential for telehealth to serve geographically isolated individuals and extend the reach of scarce resources while also emphasizing the quality and value in the delivery of health care services. This workshop summary discusses the evolution of telehealth since 1996, including the increasing role of the private sector, policies that have promoted or delayed the use of telehealth, and consumer acceptance of telehealth. The Role of Telehealth in an Evolving Health Care Environment: Workshop Summary discusses the current evidence base for telehealth, including available data and gaps in data; discuss how technological developments, including mobile telehealth, electronic intensive care units, remote monitoring, social networking, and wearable devices, in conjunction with the push for electronic health records, is changing the delivery of health care in rural and urban environments. This report also summarizes actions that the U.S. Department of Health and Human Services (HHS) can undertake to further the use of telehealth to improve health care outcomes while controlling costs in the current health care environment.

This consultation considers the collection and use of "communications data", an important technical capability that is used to protect the public against terrorism, serious crime, illegal drugs and to protect the vulnerable in society. But the capability is declining in the face of the rapidly changing communications industry. Communications data is information about a communication and does not include the content of such communications. The police, Serious Organised Crime Agency, HM Revenue and Customs and the intelligence services (and other public authorities in limited areas) can acquire communications data from communications service providers. Communications data is used extensively as evidence in courts. Access to this data is strictly controlled, as the right to privacy must be maintained. Technical changes in the communications industry means companies will offer more communications services, for voice, data and media, including TV, social networking, music, video messaging, games, text, email and internet browsing. These changes will impact on the current capability of access and use of communications data so action must be taken to maintain the existing capability. The Government does not plan for a centralised database for storing all communications data. Instead legislation will ensure that the data required is collected and retained by the communications service providers, including data from UK communications service providers and additional data relating to services provided from overseas providers (third party data). Responsibility for collecting third party data would fall on those communications providers, such as the fixed line, mobile and WiFi operators who own the network infrastructure. The problem of fragmentation would hinder investigating authorities though, so a further step requiring service providers to process the third party data and match it with their own business data where it has elements in common is proposed.

Written for communications specialists and policy makers, this book is a penetrating examination into the rapidly changing approach of states to telecommunications regulation and planning since the divestiture of AT&T in January, 1984. The editors place particular emphasis on the conjunction between the increasing state role in developing and implementing telecommunications policy and a new interest in economic development on the part of state governments. Following a discussion of the major issues surrounding telecommunications regulation and an exploration of the links between telecommunications and economic development, the experiences of nine states are considered in separate chapters. The contributors also consider telecommunications applications for improving efficiency in state government. The result is a comprehensive look at existing trends in state telecommunications regulation that will be invaluable not only to officials and legislators, but also to students of communications policy. Each of the nine state chapters includes a profile of the state's social and economic makeup, a description of the policy environment, a statement of regulatory policy, and and analysis of the relationship between telecommunications and economic development in that state. A number of chapters also include detailed case studies--among them a study of New York's Teleport, Nebraska's AGNET, and Washington State's 1985 Regulatory Flexibility Act. Based on their in-depth study of the nine states' experiences, the editors argue that states need to become better informed about the changing telecommunications environment and its potenial for improving efficiency in state government. In addition, both planning and regulation must be more related to economic development plans than they are currently in most states. Finally, the editors conclude that traditional state regulation of telecommunications companies is inadequate for establishing policy in this increasingly complex and important area.
