Infections that occur in the wound created by an invasive surgical procedure are generally referred to as surgical site infections (SSIs). SSIs are one of the most important causes of healthcare-associated infections (HCAIs). A prevalence survey undertaken in 2006 suggested that approximately 8% of patients in hospital in the UK have an HCAI. SSIs accounted for 14% of these infections and nearly 5% of patients who had undergone a surgical procedure were found to have developed an SSI. However, prevalence studies tend to underestimate SSI because many of these infections occur after the patient has been discharged from hospital. SSIs are associated with considerable morbidity and it has been reported that over one-third of postoperative deaths are related, at least in part, to SSI. However, it is important to recognise that SSIs can range from a relatively trivial wound discharge with no other complications to a life-threatening condition. Other clinical outcomes of SSIs include poor scars that are cosmetically unacceptable, such as those that are spreading, hypertrophic or keloid, persistent pain and itching, restriction of movement, particularly when over joints, and a significant impact on emotional wellbeing. SSI can double the length of time a patient stays in hospital and thereby increase the costs of health care. Additional costs attributable to SSI of between £814 and £6626 have been reported depending on the type of surgery and the severity of the infection. The main additional costs are related to re-operation, extra nursing care and interventions, and drug treatment costs. The indirect costs, due to loss of productivity, patient dissatisfaction and litigation, and reduced quality of life, have been studied less extensively.

Background : One of the most serious complications following open-heart surgery is site infection, ranging from superficial to more severe and involving deep tissue layers. It can lead to significant increases in morbidity, length of hospital stay, and cost of care. Our goal was to evaluate the incidence of sternal wound infections in a high-risk patient population utilizing a negative pressure wound dressing (NPWD) as an adjunctive therapy for 5-7 days after surgery. Methods : We retrospectively studied 400 patients in which a median sternotomy was performed. In 128 patients the NPWD was applied, while 272 of them had conventional dressing applied over a clean-closed incision. Patientspecific and procedure-related risk factors were identified for both groups and compared to determine similarity between them. Findings during hospital stay were documented, as well as those seen 30 days after surgery on follow up visit. Results : Erythema and serosanguinous fluid drainage developed in sixteen patients out of the 272 patients (5.88%) in the conventional dressing group, while only four patients out of 128 (3.13%) in the NPWD developed such complications. Seven patients out of the 272 (2.57%) in the conventional dressing group developed a superficial wound infection, while there were only three patients out of the 128 (2.34%) in the NPWD group. Deep wound infections were found only in one patient (0.37%) in the conventional dressing group. Conclusion : Despite studying a population at high risk for sternal wound complications, a trend toward fewer complications was observed in the NPWD group. When compared to the group with the conventional dressing, the difference found didn't reach statistical significance. Application of NPWD over a clean-closed incision for a period of 5-7 days in a high-risk patient population after median sternotomy can potentially reduce the risk of complications to that of a low risk group.
