Cardiovascular diseases (CVD) are increasing in epidemic proportions in developing countries. CVD already accounts for almost 10 percent of the developing world's burden of disease and is likely to become the developing world's leading cause of death. There is reason for hope, however, given that huge potential exists for applying R&D to control this emerging epidemicâ€"both in creating powerful new interventions such as vaccines and dietary supplements and in guiding behavior. In addition, a considerable body of evidence suggests that current risk-factor prevention programs and low-cost case management of CVD offer feasible, cost-effective ways to reduce CVD mortality and disability in developing country populations. Large-scale CVD control efforts are lacking, however, and thus governments and individuals are left to make choices about health and health care services without the benefit of appropriate knowledge. This report was designed to promote a policy dialogue on CVD based on informed knowledge of R&D opportunities that offer effective, affordable, and widely applicable responses in developing countries. The report examines (a) the emerging burden of CVD in developing countries, (b) the future worldwide burden of CVD, (c) current prevention and treatment of CVD in developing countries, (d) R&D to support CVD control, (e) opportunities and priorities for R&D, and the need for institutional arrangements for collaboration in facing the epidemic.

According to the World Health Organization (WHO), cardiovascular disease (CVD), which sits under the chronic disease umbrella, is the number one cause of death globally. Over time, we have witnessed different trends that have influenced the prevalence of CVD. One of the ways of decreasing CVD and its social costs and global fatalities is through influencing preventable CVD risk factors. Though many risk factors such as age and gender are not preventable, there are several effective behaviours that reduce the risk of CVD. To estimate the potential impact of various interventions on CVD, such as reducing blood pressure as a result of lowering sodium intake, or increasing awareness regarding healthy eating behaviour, we have used descriptive statistics and modelling. We estimated the impact of a gradual decrease in sodium intake on CVD mortality and morbidity in Canada (CA), United States (US), and Latin American (LA) countries. Our analysis shows that small changes in sodium intake at the population level can make an important difference in the total number of CVD events that can be prevented. Using data in Canada and France we also explored the potential role of individual decision making on daily sodium consumption. Our analysis showed that the main obstacle to consumers making healthier choices appears to be neither the availability of products, nor the price. Consumers may be more hampered by the difficulty of comparing food labels than by the availability of lower sodium products. Using Canadian data, we also examined the potential impact of having a positive family history of CVD on CVD mortality. Based on our analysis, father stroke before the age of 60 was a strong predictor for CVD mortality. Following this analysis, we used mathematical models, to improve our understanding of the impact on CVD of changes in the trend of CVD risk factors such as obesity, social and environmental influences. We investigated each of these risk factors separately, in order to have a clear foundation for more complex models. We also used a Fuzzy Cognitive Map (FCM) that considered a wide range of interactions and interrelationships between different CVD risk factors.

Regular physical activity is proven to help prevent and treat noncommunicable diseases (NCDs) such as heart disease stroke diabetes and breast and colon cancer. The global burden of chronic non-communicable diseases (NCDs), such as hypertension, diabetes and cancers, and of common mental disorders such as depression and anxiety, has a disproportionate impact on the low- and middle-income countries (LMICs) of Africa, Asia and Latin America. The pattern persists in African and Asian migrant populations in European and North American countries, despite the higher standards of living and improved health infrastructure. The consensus of experts is that pragmatic, cost-effective and sustainable interventions are required, and that these must prioritise the social determinants of NCDs as well as the social participation of affected communities. Despite the growing emphasis on the role of social processes in health system responses to chronic disease in LMICs, there has been no definitive volume that brings together LMIC perspectives on these issues. This book aims to address this major gap by presenting new conceptual and empirical perspectives on the interconnections between culture, ethnicity and chronic conditions in LMICs and their implications for research, intervention and policy.
