Proton and heavy-ion radiation therapy has been taking place now for 40 years, at many accelerator laboratories around the world, essentially all of these centers built originally for physics research. The high degree of promise shown for using these particles for treating and curing cancer has stimulated the medical community to look seriously at building dedicated accelerator facilities in a hospital setting, where more rapid progress can be made in clinical research, and development of effective treatments with these beams. In the United States, the first such facility, at the Loma Linda University Medical Center, has been in operation now for two years, and is currently treating a total of 35 to 40 patients per day. Two new projects are being designed at present, one at the Massachusetts General Hospital in Boston, Massachusetts, the second a joint project of the Lawrence Berkeley Laboratory and the University of California at Davis Medical Center in Sacramento, California. This paper will discuss accelerator and beam characteristics relevant to the proton-therapy application, and will present performance and operations characteristics for the Loma Linda facility, as well as details of the plans, process and progress towards construction of the new facilities in Boston and Sacramento.

Over the past twenty-five years, proton therapy has become more prominent worldwide. It is an important component of clinical radiation therapy for both adult and pediatric clinical care. Due to the inherent ability of protons to spare normal tissue, protons will continue to develop and become increasingly important in radiation oncology. As such, Proton Therapy - Current Status and Future Directions reviews many aspects of proton care including the application of protons in modern clinical trials. It also reviews problems associated with the migration of proton care worldwide and examines the future direction of proton care. This project was created by colleagues at IntechOpen and was carefully managed by Romina Rovan. It has been a privilege to help coordinate the text and chapters designed to acknowledge the history, footprint, and growing interest of proton care worldwide. Proton management is now embedded in the clinical trials process. In pediatric care, proton delivery is embedded with photons for the management of pediatric malignancies and adult groups have initiated proton-specific clinical trials. A proton registry has been established and outcomes are under evaluation. The Indiana University Cyclotron Facility houses three research accelerators (a 205-MeV cyclotron, a 240-MeV synchrotron, and a 500-MeV synchrotron) with space to carry out a program of fundamental and applied research including nuclear and accelerator physics, materials science, radiation biology, and proton therapy. Twelve thousand square feet associated with the 205-MeV machine is being converted into a regional proton therapy center. This center is aimed at evaluating the efficacy of treatments, and research in the development of new treatments. There will be three patient treatment rooms and an outpatient clinic. During the funding period the following activities were undertaken: construction and commissioning of the T0 Achromat; design, construction, and commissioning of the Trunk Line; design of the Energy Selection Line for all three treatment rooms; construction of the Energy Selection Line to the first treatment room; and design of the Dose Delivery System for the Large Field Line. A detailed description of each of these activities is given.
