What does the future hold for research or for the patients who have benefited from research and are now cured? etc. The first part deals with preclinical and clinical issues that relate to the disease and the second part deals with the results of recent trials and in some cases ongoing trials. It could be argued that section B Pharmacology, Section D Radiotherapy and Section E Surgery of part one belong to part two; however in these sections we have tried to focus on one discipline and in part two, we have tried to focus on the results of multidisciplinary trials. A third part includes a few critiques and editorial comments. This volume was prepared and planned for both basic and clinical oncologists. It is hoped that the basic scientists will gain some insight into the problems of their clinical colleagues and that the clinicians will be willing to read to the more basic articles. It is not a text for students (or for that matter physicians) who want all things to be laid out in `black and white'. Childhood cancer was almost always fatal before 1970. Today, 80% of ch- dren diagnosed with cancer will survive at least 5 years; 70% will be cured. However, cancer continues to be the leading cause of nonaccident-related deaths in children. More children die each year from cancer than from c- tic ?brosis, diabetes, asthma, AIDS, and congenital abnormalities combined. Nine children die from cancer every school day. Despite the fact that pediatric cancers account for only a small p- centage of the total cancer burden, more effective therapies are needed to improve mortality rates in children. Therefore, our research priority must be the development of new agents and new therapies for childhood cancer. M. D. Anderson Cancer Center has been caring for children with c- cer since its doors opened over 60 years ago. This institution has been home to several research and clinical pioneers whose innovative work has in?uenced and shaped the practice of pediatric oncology throughout the world. Notably, Drs. Wataru W. Sutow, Donald Pinkel, and Norman Jaffe have been credited with introducing curative therapies for children with leukemia and solid tumors. However, approximately 80% of new cases of cancer in children appear in mid- and low-income countries. Almost invariably, general practitioners or general pediatricians without special training in oncology will look after children with malignancies who enter the health care system in these countries. The diagnostic facilities are usually limited, as are the treatment options. The survival figures in these conditions are somewhere below 20%, while in high-income countries they are in the range of 80% for many childhood cancers. This book includes simplified therapy protocols, pain therapy and palliation, as well as ward procedures such as bone marrow aspiration/biopsies, intravenous therapy and chemotherapy drugs mixing.
