The realm of special operations forces (SOF) medicine is a unique and ever-changing one that demands specialized training for our joint SOF. Managing trauma on today’s battlefield presents a dynamic array of challenges where limited resources can be rapidly overwhelmed. An austere environment, hostile gunfire, and delays in casualty evacuation (CASEVAC) are the norms for the special operations medic. The material in this handbook was gleaned from special operations medics operating in the Global War on Terrorism and other operational environments. It should not be viewed as a substitute for the professional training and judgment of special operations medics; rather, it is designed to be a hip-pocket reference on the tactics, techniques, and procedures (TTP) of SOF-relevant tactical combat casualty care. Key Lessons Ninety percent of combat loss of life occurs before casualties ever reach a military treatment facility (MTF); treatment prior to casualty evacuation is vital. Litter carries are fundamental for good patient care; they prevent further injury and get individuals off target as soon as possible. Rehearse manual carry methods prior to deployment. Every special operations warfighter should carry a tourniquet and be thoroughly familiar with its application. When managing multiple casualties, apply the principles of triage in classifying the priority of treatment and evacuation. Rehearse and employ all of the mechanics of CASEVAC from the point of injury to the handover at a MTF. This handbook provides a number of considerations when employing medical support to SOF in combat. The challenges are numerous, but the special operations medic must deliver medical care to save Soldiers’ lives. Whilst this book is primarily compiled for nurses who care for patients with eye disease, it will be useful to other health professionals who are also interested in the holistic ca re of such patients, e.g. opticians, orthop tists, social workers and junior medical staff. Typically, a problem is identified with a desired outcome and the nursing interventions possible to achieve this. Evaluation has not been included as this is dependent on an identified time limit for outcomes to be achieved which is individual to each patient. Part One covers the general aspects of ophthalmie care in the various situations where patients are nursed, e.g. casualty, theatre, wards. Part Two is based on a medical model, where each section of the eye is studied separately; the applied physiology of that part of the eye; the more common conditions affecting it; the possible medical management of the condition and the nursing care of the patient taking into account the medical prescription. The list of medical conditions is not exhaus tive, and only a general guideline of medical management is given as far as it influences nursing care. Blood pressure and pulse are particularly inaccurate when used to identity early blood loss in trauma victims. This research supports the continued development of a system for noninvasively monitoring blood loss. We have developed a breadboard Eye Oximeter (EOX) which shines low power lasers into a subject's eye and scans the beams across the arteries and veins of the retina. The back reflected light is collected and from these signals we calculate the oxygen saturation of the blood within the retinal vessels. Prior to initiating this contract we completed swine studies that demonstrated a strong correlation between retinal venous oxygen saturation and early blood loss. During the past year, we have completed construction of a prototype Eye Oximeter, performed model eye experiments with this prototype which verity in vitro calibration, performed pilot human experiments which demonstrate significant improvements in our prototype, and begun animal testing of the device. We have also completed the development and design of a video oximeter (EOX-2) as anticipated in our statement of work. We have completed the construction of the EOX-2 breadboard and testing of the device has begun. In this Annual Report we document the studies, results and analysis which we performed under contract.
