This book presents recent efforts and new approaches to improve our understanding of the evolution of health and mortality in urban environments in the long run, looking at transformation and adaptations during the process of rapid population growth. In a world characterized by large and rapidly evolving urban environments, the past and present challenges cities face is one of the key topics in our society. Cities are a world of differences and, consequently, of inequalities. At the same time cities remain, above all, the spaces of interactions among a variety of social groups, the places where poor, middle-class, and wealthy people, as well as elites, have coexisted in harmony or tension. Urban areas also form specific epidemiological environments since they are characterized by population concentration and density, and a high variety of social spaces from wealthy neighborhoods to slums. Inversely and coherently, cities develop answers in terms of sanitary policies and health infrastructures. This balance between risk and protective factors is, however, not at all constant across time and space and is especially endangered in periods of massive demographic growth, particularly periods of urbanization mainly led by immigration flows that transform both the socioeconomic and demographic composition of urban populations and the morphological nature of urban environments. Therefore this book is an unique contribution in which present day and past socio-demographic and health challenges confronted by big urban environments are combined.

Twenty-three major databases containing historical longitudinal population data are presented and discussed in this volume, focusing on their aims, content, design, and structure. Some of these databases are based on pure longitudinal sources, such as population registers that continuously observe and record demographic events, including migration and family and household composition. At STIAS, the ‘Health in Transition’ theme includes a programme to address the epidemic rise in the incidence of non-communicable diseases (NCDs) such as Type 2 diabetes, hypertension, obesity, coronary heart disease and stroke in Africa. The aim is to advance awareness, research capacity and knowledge translation of science related to the Developmental Origins of Health and Disease (DOHaD) as a means of preventing NCDs in future generations. Application of DOHaD science is a promising avenue for prevention, as this field is identifying how health and nutrition from conception through the first 1 000 days of life can dramatically impact a developing individual’s future life course, and specifically predicate whether or not they are programmed in infancy to develop NCDs in later life. Prevention of NCDs is an essential strategy as, if unchecked, the burden of caring for a growing and ageing population with these diseases threatens to consume entire health budgets, as well as negatively impact the quality of life of millions. Africa in particular needs specific, focussed endeavors to realize the maximal preventive potential of DOHaD science, and a means of generating governmental and public awareness about the links between health in infancy and disease in adult life. This volume summarizes the expertise and experience of a leading group of international scientists led by Abdallah Daar brought together at STIAS as part of the ‘Health in Transition’ programme.

Early modern Naples has been characterized as a marginal, wild and exotic place on the fringes of the European world, and as such an appropriate target of attempts, by Catholic missionaries and others, to ’civilize’ the city. Historiographically bypassed in favour of Venice, Florence and Rome, Naples is frequently seen as emblematic of the cultural and political decline in the Italian peninsula and as epitomizing the problems of southern Italy. Yet, as this volume makes plain, such views blind us to some of its most extraordinary qualities, and limit our understanding, not only of one of the world's great capital cities, but also of the wider social, cultural and political dynamics of early modern Europe. As the centre of Spanish colonial power within Europe during the vicerealty, and with a population second only to Paris in early modern Europe, Naples is a city that deserves serious study. Further, as a Habsburg dominion, it offers vital points of comparison with non-European sites which were subject to European colonialism. While European colonization outside Europe has received intense scholarly attention, its cultural impact and representation within Europe remain under-explored. Too much has been taken for granted. Too few questions have been posed. In the sphere of the visual arts, investigation reveals that Neapolitan urbanism, architecture, painting and sculpture were of the highest quality during this period, while differing significantly from those of other Italian cities. For long ignored or treated as the subaltern sister of Rome, this urban treasure house is only now receiving the attention from scholars that it has so long deserved.
