A statistical method was developed and used to perform binary and multiclass classification of the normal, benign and malignant tissue types. Receiver operating characteristic (ROC) analysis was employed to determine the optimal model for classification yielding maximum sensitivity and specificity. The optical channel-based classification gave better results compared to technique-based classification. The neoplastic and non-neoplastic tissues were discriminated with 100% sensitivity and 100% specificity. In multiclass classification, the benign was detected with 100% sensitivity and 95.2% specificity and the malignant was classified with 100% sensitivity and 71.4% specificity. In summary, an optical diagnostic model for unbiased classification of normal, benign and malignant lesions was developed. The diagnostic optical biomarkers identified, helped to perform label-free imaging. This multi-modal imaging approach showed the potential to serve as a reliable screening tool for pathologists to objectively identify the diseased cases.

The goal of this thesis is to study the potential of optical spectroscopy as a clinical diagnostic tool for melanoma and nonmelanoma skin cancer. Skin cancer is the most common cancer in the United States. Like most cancers, early diagnosis and treatment improves patient prognosis for both melanoma and nonmelanoma skin cancer. However, current "gold standard" for diagnosis is invasive, costly and time-consuming. A diagnostic procedure consists of a clinical examination of the suspicious lesion, followed by biopsy and histopathology, with an additional turnaround time of approximately one week. There is a need for an accurate, objective, noninvasive, and faster method to aid physician in diagnosing cancerous lesions, increasing diagnosis accuracy while preventing unnecessary biopsies. We propose Spectral Diagnosis, a system capable of noninvasive in vivo spectroscopic examination of human skin. The research objectives are: (1) Probe pressure effects on in vivo spectroscopy measurements of human skin, (2) Clinical trial of Spectral Diagnosis, (3) Design, construction, and characterization of a confocal Raman microspectroscope. Spectral Diagnosis utilizes an optical fiber probe that transmits and collects optical spectra in contact with the suspected lesion. We identified short term and light probe pressure effects to be minimal on diagnostic parameters, and should not negatively influence diagnostic performance. We conducted a clinical trial at the University of Texas MD Anderson Cancer Center, and our results show that principal components from three spectroscopy modalities (diffuse reflectance spectroscopy, laser induced fluorescence spectroscopy, and Raman spectroscopy) provide excellent melanoma and nonmelanoma skin cancer diagnosis. We also constructed and characterized a Raman microspectroscope, with the goal of developing a physiological-based fitting model to better understand the analysis of in vivo Raman spectroscopy data from human skin tissue.

Current surgical oncology faces great challenges due to the lack of accurate intraoperative image-guidance and rapid histopathological assessment. This leads to incomplete tumor resections and cancer recurrences. We have developed the fluorescence goggle system to overcome these limitations. The fluorescence goggle is a real-time intraoperative imaging and display system that, when combined with fluorescent molecular probes, can identify the extension of tumors and guide surgical resections. It is compact, wireless, wearable, battery-operated, and allows for hands-free imaging by surgeons. Unlike conventional imaging instruments, the fluorescence goggle directly displays fluorescence information on its eyepieces. We have developed several generations of fluorescence goggle based on CCD and CMOS imaging technologies, as well as see-through display technologies. We have applied the fluorescence goggle to aid cancer staging and surgical resection of breast, liver and brain cancer, in both small animals and humans. To facilitate pathological diagnosis, we have developed a complementary fluorescence-polarization dual-modal microscope and a division of focal plane polarized light microscope. These novel microscopes greatly assist rapid histopathological assessment of cancer. The combination of real-time image-guided surgeries using the fluorescence goggle and ex vivo histopathology using the multimodal microscopes improves surgical planning and patient outcomes.
