Global Orthopedics: Caring for Musculoskeletal Conditions and Injuries in Austere Settings was conceived and written to be a unique reference for surgeons working in resource-limited environments. The first sections provide historical background, global public health perspectives of orthopedics, the role of culture, and a broad discussion of clinical topics that orthopedic surgeons rarely deal with in high-resource settings but that affect orthopedic care. Adult and pediatric trauma are presented in an anatomical format for easy reference, with a focus on the natural history and the best treatment methods within existing limitations. The chapters on musculoskeletal infections provide a focused discussion about these common debilitating conditions that is unavailable in any other single modern text. The non-infectious pediatric conditions section has been written for the non-specialist to handle selected developmental and early childhood orthopedic problems commonly seen in low-resource settings. Detailed chapters on reconstruction surgery, tumor management, amputations, and the orthopedic needs in the face of conflicts and natural disasters round out the text.

A critical problem in resource-scarce countries across the globe is the shortage of appropriately trained health care providers. According to the World Health Organization, the current global health workforce shortage of 7.2 million providers is estimated to increase to 12.9 million by 2035. This disproportionately affects resource-scarce countries, denying basic health care to millions and limiting access to life-saving treatments. Due to limited resources in these countries, not enough health professionals receive training, few have the opportunity for continuing education, and the ability to develop or implement educational programs and curricula is constrained. Additionally, many existing providers choose to emigrate in pursuit of professional advancement opportunities, contributing to the overall shortage of qualified health care providers in these environments. Efforts to strengthen health workforce capacity not only increases access, safety and availability of care, but is critical to building resilient health systems capable of caring for the world’s neediest populations. This requires not only cultivating new health care providers, but also providing ongoing professional development to retain and support current providers, advancing the level of practice in accordance with current clinical science, cultivating educators, and enhancing training curricula. It is critical also to contribute to the limited body of research documenting the effectiveness and impact of various models of collaborative education and partnership to improve health worker training and retention. This Research Topic examines strategies for building health workforce capacity through the prism of educational partnerships, offering significant examples of effective models of international collaborative education as well as insight and guidance on the structure and operation of successful global partnerships. Collectively, the 31 articles accepted and included in this eBook represent a diversity of health professions and geographies across academic, non-governmental organizations and other global partnership forms. The published manuscripts highlight various elements of partnerships with several consistent themes emerging: capacity building, local empowerment, mutual trust and respect, long-term commitment, equity, collaboration, and the importance of integrating theory and practice, for a balance of academic and clinical development. The manuscripts provide examples of partnership and educational programs that are in the formative, early stages of implementation and others which have been sustained long term, some for decades. The following eBook is divided into two parts, with each part broken down into sections. Part I of the eBook includes 18 manuscripts that showcase long-term educational programs that strongly exemplify multiple, foundational aspects of international partnerships in education including mutual collaboration and project management, empowerment of host partners to lead and sustain programs, and capacity building. While individual manuscripts included in Part I look broadly at multiple aspects of successful, international partnerships in education, Part II manuscripts focus intently on one-two elements. Part II includes 13 articles that highlight partnership through short- rather than long-term educational initiatives as well as program development and broad academic partnerships. This Research Topic was sponsored by Health Volunteers Overseas – a United States based non-profit that collaborates with over eighty international universities and health institutions to send volunteer health professionals to low-resource countries to provide continuing education, train the trainer courses, professional support, and consultation on academic program and curricula development.

This text was developed as a book aimed at surgeons and allied health professionals that provides an introduction to the unmet needs , epidemiological, socioeconomic and even political factors that frame Global Surgery. Following upon an understanding of these issues, the text is a practical guide that enables the reader on several levels: to work cross culturally , build relationships and negotiate the logistical challenges of bringing surgical care to low resource settings; to develop an approach to the management of various clinical conditions that would be unfamiliar to most “western” surgeons. Global Surgery is a recently coined term that encompasses many potential meanings. Most would agree that it focuses on the growing recognition of the crisis of access to quality surgical care in low resource settings. Such scenarios exist on every continent. Increasingly surgeons, allied health professionals (NGO), Public Health / Health Policy professionals as well as governmental and non-governmental organizations are engaging in this field. Many surgeons have an interest in Global Health and a desire to become involved but feel ill equipped to do so and unsure where to start. Global Surgery: The Essentials serves as a ready resource to equip surgeons to manage clinical scenarios that lie beyond the scope of their training or current practice but that they would reasonably be expected to encounter in the field.

As the healthcare landscape evolves towards value-based treatment models, healthcare providers will be forced to find ways to deliver healthcare in a cost-effective, resource mindful way that provides good care, all the while maintaining appropriate patient satisfaction. Telemedicine offers a way to achieve this goal, in both rural and urban settings and with a varied and diverse patient population - not to mention during global health emergencies, where in-person visits and consultations are not ideal. This book will serve as an introduction to telemedicine and digital health for the orthopedic and sports medicine provider. It will provide a general overview of telemedicine as well as specific suggestions and recommendations: where and how to get started, how to implement a telemedicine program, how to do research in telemedicine, and how to develop clinical guidelines and best practices for work in telemedicine. Specific chapters cover important nuts-and-bolts topics like regulation and licensing, billing and coding, and ethics and etiquette. Suggestions and considerations for provider-to-provider, direct-to-consumer, and school-based telemedicine service are likewise presented. Finally, insights into global telemedicine implementation and research are detailed. While describing specific applications to orthopedic and sports medicine practices, Telemedicine in Orthopedic Surgery and Sports Medicine will cater to any clinician – from the individual solo practitioner to the C-suite level executive – who has a vision for implementation of telemedicine across an entire health system.
