Background: Atrial fibrillation is a growing problem among the population worldwide. For ablation of atrial fibrillation, fluoroscopic imaging is generally essential for visualization purposes. The usage of x-rays can lead to various adverse effects like cancer.The aim of this master ́s thesis was to evaluate, if the use of electroanatomic 3D imaging technologies leads to a reduction in fluoroscopic exposure, compared to cryoablation.Methods: A retrospective, single-center study was conducted to compare the use of RF ablation (including a 3D mapping system) with cryoablation in pulmonary vein isolation. The primary endpoint is the mean dose area product in μGy*m2. Secondary endpoints are comparisons of body mass indices, the fluoroscopic exposure time (min), an analysis of the subgroups (paroxysmal and persistent AF) and the effect of an ultra low dose fluoroscopy program.Results: Of the 284 patients that were encompassed in the study, 167 underwent pulmonary vein isolation with RF ablation and the use of a 3D mapping system. 117 were treated with cryoablation to achieve pulmonary vein isolation.The use of RF ablation with a 3D mapping system resulted in a dose area reduction of 51.67% (2369.73 μGy*m2 vs. 4903.51 μGy*m2), compared to cryoablation, including all procedures between August 2015 and December 2017.Conclusion: The use of RF ablation including imaging with a 3D mapping system resulted in a reduction of the mean dose area product in the ablation of atrial fibrillation, compared to the usage of cryoablation.*****Background: Atrial fibrillation is a growing problem among the population worldwide. Catheter ablation is a treatment for patients with heart rhythm disturbances (cardiac arrtiythmias) called tachycardias. Tachycardias cause symptoms that degrade the quality of life of individuals and are a life-long medical problem. Some of them are common medical problems (such as atrial fibrillation) and many begin at a young age with the potential for life-long morbidity. Certain tachycardias can be life threatening. Drug therapy to control these tachycardias is often ineffective or causes intolerable side effects. Presently, catheter ablation delivered by radiofrequency energy (RPA) is the predominant procedure used for the treatment of tachycardias and may be curative or palliative. This is the first synthesis of research studies and economic evaluations of RFA in Canada. For the following conditions, RFA is associated with a high procedural success rate and a low rate of complications within two years of follow-up: a. Paroxysmal supraventricular tachycardia (PSVT) secondary to an accessory pathway. b. PSVT secondary to atrioventricular node re-entry c. Atrial flutter d. Focal and re-entrant atrial tachycardias. For the following conditions, catheter ablation is still within the research domain: a. Atrial fibrillation; b. Ventricular tachycardia (VT) in the setting of structural heart disease. In adult patients with either symptomatic PSVT or VT patients with implantable defibrillators who experience frequent recurrences, RFA is both more effective and less costly than drug therapy options. For these patients, RFA costs within US $21,000 (C $33,000) per quality-adjusted-life-year gained. For all the different types of ablation procedures, there is a paucity of high quality outcome studies comparing ablation with altemative therapies.
