Family planning programs have been highly successful over the past 30 years in providing women in developing countries with desired access to contraceptive services and helping to reduce fertility rates. Notwithstanding this success, there is still an urgent need for these programs. The world's population is increasing, with annual population growth still approximately 80 million people. Nearly all of this growth is occurring in developing nations, where fertility rates remain relatively high. This high fertility runs counter to the preferences expressed by millions of women, who actually want to have smaller families. Family planning programs are also desirable because they are associated with a range of other benefits, most notably improvements in women's and children's health. Host countries provide about 60 to 75 percent of funding for family planning. However, funding and technical assistance from donor nations, especially the United States, have been crucial to the past success of family planning programs and are equally important for strengthening and expanding program efforts to meet future challenges.

This paper addresses the key elements and changes in population policies and programs, including the strength of program effort, from 1972 to 1982. Using a new scale with thirty items to measure family planning program effort, the presentation is based on an analysis of over 300 questionnaires received from ninety-three countries. Attention is given to measurement problems and change in program effort 1972-82 (including findings by country and regional differences). This study shows that a great deal of family planning program effort exists in a small number of countries; moderate effort occurs in a larger number of countries; and weak or very little effort is found in an even greater number of countries, including many in the Middle East, North Africa and sub-Saharan Africa. Concerning change over time, between 1972 and 1982, there was a modest increase in program effort in more than half of the nearly 100 countries studied and a substantial increase in program effort in more than a third.

A Working Group on Administrative Aspects of Family Planning Programmes met in Bangkok, Thailand, March 16-24, 1966, to discuss problems peculiar to family planning program administration. The meeting was oriented toward operational problems that might be encountered in implementing programs at the local level; technical problems dealing with training, education, communications, and the manufacture of birth control devices were mentioned only in relation to family planning administration. Participants included 23 people from the 10 member countries of the Economic Commission for Asia and the Far East, 12 experts in family planning problems in that area, and representatives from several international organizations. The agenda, summarized in this publication, included: 1) reports on family planning programs in member countries; 2) setting targets for family planning programs; 3) assessing and meeting administrative requirements in personnel, recruitment, training, supplies, and transport; 4) budgets and timetables; 5) making cooperative arrangements with government and voluntary agencies; 6) specific principles in program implementation in regard to maintaining flexibility in the allocation of resources and the delegation of discretionary powers to personnel at different levels; 7) evaluation for the administrator of the effectiveness of the program; and 8) dissemination of knowledge and experience between workers within the country and between countries in the region.
