The present study aims to increase a woman's knowledge of: (1) the factors that determine genetic predisposition to breast/ovarian cancer, (2) personal family history and other risk factors, (3) the benefits and drawbacks of genetic testing for breast/ovarian cancer, (4) the range of surveillance and preventive behaviors available and, (5) the process of genetic testing. Participants will be 200 women who have contacted the Cancer Information Service (CIS) requesting information on inherited breast/ovarian cancer. Women will be randomly assigned to a standard or enhanced intervention testing the effectiveness of the CIS in increasing a woman's knowledge of inherited disease, risk assessment and genetic testing. As planned, this first year was dedicated to a formative evaluation of the informational/emotional needs of women pursuing risk assessment services. The structured interviews and focus groups conducted with women from the lay population, women at high risk and genetic counselors informed the development of the enhanced intervention. The interviews/groups supported previous research demonstrating that women are unprepared for and have limited knowledge about the process and outcomes of risk assessment for genetic predisposition to breast/ovarian cancer. A comprehensive training program and formalized standard protocol will ensure competence and consistency when accrual begins in the Pall.

This lack of information leads them to feel unprepared for risk assessment/genetic testing, if they choose to seek it. This pilot study developed an enhanced intervention to increase a woman's knowledge of: 1) the factors that determine a genetic predisposition to breast/ovarian cancer, 2) personal family history and other risk factors, 3) the benefits and drawbacks of genetic testing for breast/ovarian cancer, 4) the range of surveillance and preventive behaviors available, and 5) the actual process of risk assessment/genetic testing. Participants were 279 women who contacted the Atlantic Region of the National Cancer Institute's (NCI) Cancer Information Service (CIS) requesting information about risk for breast/ovarian cancer and/or information about risk assessment services and genetic testing. Women were randomly assigned to either the standard or enhanced intervention. The study, comparing the standard and enhanced interventions, tested the effectiveness of the CIS in increasing a woman's knowledge of inherited breast/ovarian cancer and the process of risk assessment/genetic testing as well as her sense of preparation and intention to pursue such services.

These proceedings of a workshop presented to the Institute of Medicine's (IOM) National Cancer Policy Forum on March 30, 2007, are the result of forum discussions about genetic testing and counseling at its meetings on June 16 and October 30, 2006. Those discussions, led by forum members Betty Ferrell and Patricia Ganz, noted that genetic testing and counseling are becoming more complex and important for informing patients and families of risks and benefits of certain courses of action, and yet organized expert programs are in short supply. The subject matter involves not only the scientific and clinical aspects but also workforce and reimbursement issues, among others. Drs. Ferrell and Ganz proposed that the forum could provide a useful review of the various important implications of these issues by holding and reporting a workshop on the subject. They volunteered to work with staff to organize and lead such a workshop. The agenda for the workshop is reproduced in the appendix to these proceedings. Increasingly, the cultural beliefs and values of women are being recognized as important factors in genetic counseling for breast cancer susceptibility. Despite recommendations to increase the cultural sensitivity of genetic counseling, such programs have not been developed or evaluated. The objectives of this study are to develop a Culturally Tailored Genetic (CTGC) protocol for African American women and evaluate its impact on decision-making and satisfaction about BRCA1/2 testing, quality of life, and cancer control practices. A secondary objective of this study is to identify African American women who are most and lest likely to benefit from CTGC vs. SGC. The key research accomplishments achieved during the past year include continuing subject recruitment, completing genetic counseling and education, and generating peer-reviewed manuscripts. The results generated during the past year demonstrate that it is possible to enroll African American women into genetic counseling research. Our findings also demonstrate that African American women at increased risk for hereditary breast cancer report positive attitudes about the benefits of genetic testing; however, these attitudes may not translate into high levels of interest in testing. CTGC may be one strategy for addressing beliefs about genetic testing and facilitating testing decisions among African American women.

Download Acceptance of Referral for Cancer Risk Counseling in Clinical Populations: Variables Predicting Follow-Up at a Cancer Genetics Program Book in PDF, ePub and Kindle
This study was designed to demonstrated the utility of brief quantitative risk assessment in a breast biopsy clinic setting as a method of referral to cancer risk counseling. We are examining factors that influence the decision to undergo cancer risk counseling after the referral is made. Computerized risk assessment was performed on 120 women undergoing breast biopsy. Questionnaires measuring knowledge and attitudes about breast cancer, cancer risk counseling, and genetic testing were completed, and psychological assessments were done. Fifty-three percent had risk levels warranting referral and were followed to assess referral uptake. Although only one woman followed through with a referral appointment, 46% stated that they would definitely consider genetic counseling in the future. Primary reasons cited for declining referral were receipt of sufficient risk information from the brief risk assessment (40%) or from their doctors (49%), perceived "low risk" (40%), and no family history of breast cancer (31%). Objective risk, as calculated by statistical models may not be a sufficient motivation for seeking cancer risk counseling at a genetic-based program. Subjects cited a precipitating event such as a cancer diagnosis as a primary motivating factor, therefore the study has been modified and is continuing in a diagnosed treatment population.
