Oral health-related quality of life (OHRQoL) instruments have being widely used in oral health studies. "Children and teenagers with Osteogenesis Imperfecta (OI), may have their Oral Health-Related Quality of Life (OHRQoL) affected by the oral traits of the disease. However, there is little that we know of OHRQoL and its determinants amongst this population. The aim of this thesis is to investigate the extent of associations between disease-related oral conditions and OHRQoL in children and adolescents having OI.Aim: To estimate the extent of association between oral traits of OI and OHRQoL amongst children and adolescents suffering from OI. Methodology: Children and adolescents aged 8-14 years were recruited in the context of a multicenter longitudinal study (Brittle Bone Disease Consortium) that enrolls individuals with OI in 10 centers across North America. Data on socio-demographic factors, and medical history were collected using a questionnaire. OHRQoL was assessed using the Child Perceptions Questionnaire (CPQ) versions for 8 to 10-year-olds (CPQ8-10) and for 11 to 14-year-olds (CPQ11-14). A comprehensive oral evaluation was performed at each center by calibrated dentists. A multivariate ordinal logistic regression was employed to adjust for the potential confounders.Results: A total of 138 children and adolescents (62% girls) diagnosed with OI types I, III, IV, V and VI (n=65, 30, 37, 4 and 2, respectively) participated in the study. Caries experience (adjusted-DFT) and oral hygiene (OHI-S) were not statistically significantly different between OI types both in children and adolescents. Amongst all participants 34% (n=47) had dentinogenesis imperfecta (DI), 50% (n=69) had class III dental malocclusion, 36% (n=50) had posterior cross-bite and 28% (n=39) had lateral open-bite. The prevalence of aforementioned traits are predominantly higher amongst patients having OI type III. The only statistically significant determinants of negative impact on oral health-related quality of life was posterior cross-bite in the adolescent group with having the strongest association with functional limitation domain. Having posterior crossbite increases the odds of having higher grades of functional limitations by a factor of 6.1 (95% CI: 1.7- 22.6). Accordingly, having posterior crossbite increases the odds of having lower OHRQoL by 5.9 (95% CI: 1.6- 20.9) times amongst adolescents. No statistically significant association was observed in the children's group. Conclusion: Regardless of OI type, teenagers with posterior cross-bite experience significantly lower OHRQoL than those without. " --

Abstract: "Quality of life" is a dynamic construct. Broadly defined, it is an individual's satisfaction with the major areas of his/her life. It is associated with multiple variables and subject to change over time. An objective of this investigation was to evaluate if malocclusion and other oro-facial conditions affect oral health-related quality of life (OHRQoL). A further goal was to evaluate if changes in OHRQ0L occur with orthodontic treatment. A final aim was to evaluate the correlation between child and caregiver reports of OHRQoL. A convenience sample of 11-14-year-old children and their caregivers was recruited: pediatric group (N=64); orthodontic group (N=68); and cleft group (N48). Subjects completed OHRQ0L questionnaires at enrollment, 6-months and 12-months. The effects of group, time, and group X time were investigated using factorial ANOVA and Tukey-Kramer procedure, and the correlation between child and guardian responses was evaluated by comparing 31 questions common to both questionnaires utilizing the Pearson product-moment procedure. Group and time had significant effects on child OHRQ0L scores, and a significant interaction between group and time was found for the child subjects' 'emotional wellbeing' domain. Post hoc tests revealed significant differences in OHRQoL between the cleft groups and the orthodontic and pediatric dental groups across time. They also revealed significant changes in the orthodontic group's OHRQoL at the 12-month time period. Parents perceived oral and orofacial conditions to have significant impacts on child OHRQoL, with the parents of the cleft groups reporting their children to have significantly worse OHRQ0L compared to the reports of the orthodontic and pediatric dental parents. Parents also viewed their children's 'emotional well being' to change with time and the interaction of group and time to significantly impact 'functional limitation.' Post hoc tests revealed parents of the orthodontic group to report significantly more functional limitations at the 6-month time period when compared to baseline. Finally, Pearson product-moment procedure revealed a significant, positive relationship between caregiver and child responses to the OHRQoL questionnaires. Results from the present investigation suggest that the OHRQ0L of children is variable and influenced by oral condition, that OHRQoL improves during the first year of orthodontic care, and that parent perceptions of child OHRQoL is positively correlated with child reports.
