ECMO, or extracorporeal membrane oxygenation, is an advanced life support technique that provides cardiac and pulmonary support similar to cardiopulmonary bypass. ECPR (extracorporeal cardiopulmonary resuscitation) is the rapid deployment of VA-ECMO when conventional cardiopulmonary resuscitation fails to provide return of spontaneous circulation. Evidence in the literature is sparse, but with expanding reported applications, ECPR has shown promise to improve outcomes of cardiac arrest. ECPR is superior to conventional CPR for both survival and neurologic outcomes. ECPR has been successfully used to manage arrests secondary to cardiac and non-cardiac causes. Arrests secondary to primary cardiac causes have the best overall outcome. Other determinants of outcomes of ECPR include duration of low flow state and on-ECMO complications. A narrow list of ECPR contraindications exists, and includes severe neurologic injury and irreversible primary disease process. Various complications can occur with ECPR, and include mechanical, cardiovascular, pulmonary, hematologic, renal, and neurologic complications. Neurologic complications are the most serious, and significantly affect mortality or quality of life. ECPR is a nascent field, and substantial work remains to be done to optimize its application. Given the small number of patients at each institutional level, this is a field ripe for collaborative work and rewarding results.

Extracorporeal cardiopulmonary resuscitation (ECPR) is a method of resuscitation in which venoarterial (VA) extracorporeal membrane oxygenation (ECMO) is initiated during refractory cardiac arrest. ECPR promises to enhance outcomes after cardiac arrest by minimizing neurological deficits, stabilizing the patient for early reperfusion and hypothermia, and serving as a bridge to treatment or transplant. ECPR must be initiated according to structured guidelines and protocols, which are based on the patient’s age, comorbidities, code status, neurological baseline, no flow time, and low flow time. If a patient achieves return of spontaneous circulation on ECMO, the patient will receive post cardiac arrest care which includes but is not limited to therapeutic hypothermia, early reperfusion, intra-aortic balloon pump insertion, tight glycemic control, and low ventilation. While ECPR has been shown to improve outcomes, multiple complications including bleeding, infection, renal failure, limb ischemia, and stroke can result from the treatment. Nurses play a key role in monitoring these critical patients and achieving therapeutic outcomes. As ECPR is expensive, carries high risk of complications, and can not always be performed under informed consent, thus there are ethical implications. A review of the literature indicates that low flow time, age, percutaneous intervention, and sustained ventricular fibrillation are independent factors that directly impact patient outcomes. With advances in ECPR and its use in the clinical setting, it is evident that randomized control trials and uniform ECPR protocols and guidelines are essential to improve evidence base practice and patient outcomes.

We present a case of a 15-year-old boy who developed pulseless electrical activity(PEA) in the school gym, after 6-minutes ride. He received effective bystanderCPR performed by his teacher. Subsequent advanced CPR could not achieve asustained return of spontaneous circulation (ROSC) and refractory cardiac arrestwith PEA persisted.On hospital arrival VA-ECMO was established during active CPR after 95 min ofcardiac arrest (time from OHCA to door: 75 min; time door to VA-ECMO: 20 min).After starting Extracorporeal life Support (ECLS) an adequate systemic circulationwas restored and sustained ROSC was obtained. Despite the recovery of organperfusion, the patient showed multiple organ failure: coma, pulmonary edema,acute renal failure, metabolic acidosis, and troponin increase. Echocardiographyshowed severe left ventricle systolic dysfunction with global hypokinesia andapparently normal coronary hosts. Under ECLS we assisted to a complete recoveryof cardiac, respiratory, and renal function. After sedation withdraw, the patientperformed simple orders without motor deficits.An angiography CT scan, performed to evaluate the aetiology of the CA, revealedan anomalous origin of the left coronary artery from the right coronary sinus withsevere left main stenosis. The congenital defect was surgical repaired and ECLSmaintained until full recovery (12 days). The patient was discharged in the 57th daywith a good recovery.In refractory cardiac arrest, ECPR had several advantages, including successfulROSC, maintain organ perfusion during recovery of native cardiac output, earntime to perform diagnostic tests and apply surgical and medical treatments.

Abstract: ntroduction: Extracorporeal cardiopulmonary resuscitation (ECPR) might be a lifesaving therapy for patients with cardiac arrest and no return of spontaneous circulation during advanced life support. However, even with ECPR, mortality of these severely sick patients is high. Little is known on the exact mode of death in these patients. Methods: Retrospective registry analysis of all consecutive patients undergoing ECPR between May 2011 and May 2020 at a single center. Mode of death was judged by two researchers.
