Cerebral Palsy in Infancy is a thought-provoking book which introduces a new way of thinking on the development and use of interventions. At the end of September 1984 Dr. M. Velickovi~ and Prof. H. Prechtl with the help of the International Cerebral Palsy Society organized an international conference on "New develop ments in the assessment of early brain damage" in Bled, Yugo slavia. I was invited as a speaker at this conference and I went there, curious, but without too much knowledge about the central theme: cerebral palsy. During the conference and a satellite meeting I became impressed by the quality of various contributions and also by the great variety of problems related to the etiology, early diagnosis, management and psychosocial aspects of cerebral palsy. Today, in many areas of biology and medicine, progress seems to require concentration on a very narrow field. As a consequence many conferences are highly specialized and most (young) scientists rightly consider this as most useful for their own work. On the other hand the care of patients and counselling of parents and other close relatives require a multidisciplinary approach. Also, advances in the study of complicated un resolved medical biological pr'oblems are often made unex pectedly by using ideas, theories, approaches or methods from other disciplines.

This volume contains contributions that are interdisciplinary and inter national. The editors believe this is an especially timely and promising enterprise, for both sources of diversity are needed for improving our abilities to identify the young child at risk and to prevent disability. In terms of diSciplines, the volume brings together papers by health care providers (such as pediatricians and public health nurses) as well as educators and psychologists. Each of these groups works in dissimilar settings and faces dissimilar problems: Health care providers seek sim ple identification procedures for use in busy primary care settings; psy chologists emphasize well-constructed research designs; and educators reflect the need for early identification and education. Each of these spe cialist groups has something to offer the other, but too often each tends to limit its publications and readings to its own discipline, thus failing to capitalize on a wider scope. of knowledge and practice. We hope that this selection of papers will allow all readers addressing the early iden tification of children at risk to generate a more integrated interdiscipli nary perspective. The aim of this Research Topic was to collate articles describing prediction of outcomes of pre- and perinatal lesions leading to cerebral palsy, basic research in animal models and human subjects, and ideas for, and trials of, interventions in the first two years of life. CP arises from insults to the sensorimotor cortex, subcortical axon tracts and subplate. The aetiology is complex and often multifactorial. The outcome is not simply a loss of voluntary control due to disruption of descending pathways, but also involves abnormal development of reflex and corticospinal circuitry. CP may be viewed as aberrant plasticity in response to a lesion, indeed, abnormalities in movement are subtle at first but develop subsequently. It is misleading to suppose that developmental mechanisms are self-reparative. The challenge is to understand activitydependent fine tuning of neural circuitry during normal development and to find how to promote desirable plasticity whilst limiting undesirable effects following developmental lesions. However, before proposing interventions, we have to develop our ability to predict the severity of neonatal insults. We solicited a variety of articles, including long and short reviews, original research and opinion pieces, from both basic scientists and clinicians. Likewise we, as editors, have complementary knowledge and experience in this area. Anna Basu is an academic pediatric neurologist and Gavin Clowry is a developmental neuroscientist.
