Massive experimental, computational, and clinical studies have been performed worldwide, and are still ongoing, to understand and characterize Covid-19 molecular basis and transmission mechanisms, to develop diagnostics and vaccines, and to search for small chemical drug candidates and therapeutic proteins and peptides. Impressive results have been obtained for transmission control and vaccines so far, but what is the status of the other therapeutic options? The crisis has exposed different types of weaknesses in biomedical research in many countries. What can we learn from this crisis in the field of drug discovery and development so as to emerge stronger? The Covid-19 crisis has revealed the strengths of modern drug discovery and vaccine development but also exposed different types of weaknesses that would need to be addressed to be better prepared for a possible next global health crisis. These challenges/weaknesses/obstacles are of very different nature and as such hard to tackle. For instance, in addition to the inherent challenging nature of the scientific discovery, some scientists have mentioned insufficient local/national coordination and observed a fragmented drug discovery research, others have underlined a lack of coordination between the academic system and the private sector, an inadequate international global coordination and cooperation, others highlighted insufficient infrastructures, inappropriate financial supports, limited discussions between the scientific community performing the research and the public health authorities, decision makers and the society, while some remarked insufficient education/training about drug discovery and development leading to confusion… The crisis also raised important scientific questions about the technologies that would need to be used during an emergency situation, combined, integrated, or developed so as to accelerate the identification of small molecule drug candidates and of therapeutic peptides/proteins.

In response to the coronavirus disease 2019 (COVID-19) pandemic and the societal disruption it has brought, national governments and the international community have invested billions of dollars and immense amounts of human resources to develop a safe and effective vaccine in an unprecedented time frame. Vaccination against this novel coronavirus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), offers the possibility of significantly reducing severe morbidity and mortality and transmission when deployed alongside other public health strategies and improved therapies. Health equity is intertwined with the impact of COVID-19 and there are certain populations that are at increased risk of severe illness or death from COVID-19. In the United States and worldwide, the pandemic is having a disproportionate impact on people who are already disadvantaged by virtue of their race and ethnicity, age, health status, residence, occupation, socioeconomic condition, or other contributing factors. Framework for Equitable Allocation of COVID-19 Vaccine offers an overarching framework for vaccine allocation to assist policy makers in the domestic and global health communities. Built on widely accepted foundational principles and recognizing the distinctive characteristics of COVID-19, this report's recommendations address the commitments needed to implement equitable allocation policies for COVID-19 vaccine.

The United States and the world were unprepared for the COVID-19 pandemic, despite decades of warnings highlighting the inevitability of global pandemics and the need for international coordination. The failure to prioritize and adequately fund preparedness and effectively implement response plans has exacted a heavy human and economic price, and the crisis is not yet over. Emerging and reemerging infectious diseases are a threat to global and national security that neither the United States nor the world can afford to ignore. This Task Force proposes a comprehensive strategy that includes institutional reforms and policy innovations to help the United States and the multilateral system perform better in this crisis and when the next one emerges. Without increased U.S. leadership on and adequate investment in pandemic preparedness and response, the United States and the world will remain unnecessarily vulnerable to epidemic threats. The Council on Foreign Relations sponsors Independent Task Forces to assess issues of current and critical importance to U.S. foreign policy and provide policymakers with concrete judgments and recommendations. Diverse in backgrounds and perspectives, Task Force members aim to reach a meaningful consensus on policy through private deliberations. Once launched, Task Forces are independent of CFR and are solely responsible for the content of their reports. Task Force members are asked to join a consensus signifying that they endorse the general policy thrust and judgments reached by the group, though not necessarily every finding and recommendation. Each Task Force member also has the option of putting forward an additional or a dissenting view.

The ultimate guide for anyone wondering how President Joe Biden will respond to the COVID-19 pandemic—all his plans, goals, and executive orders in response to the coronavirus crisis. Shortly after being inaugurated as the 46th President of the United States, Joe Biden and his administration released this 200 page guide detailing his plans to respond to the coronavirus pandemic. The National Strategy for the COVID-19 Response and Pandemic Preparedness breaks down seven crucial goals of President Joe Biden's administration with regards to the coronavirus pandemic: 1. Restore trust with the American people. 2. Mount a safe, effective, and comprehensive vaccination campaign. 3. Mitigate spread through expanding masking, testing, data, treatments, health care workforce, and clear public health standards. 4. Immediately expand emergency relief and exercise the Defense Production Act. 5. Safely reopen schools, businesses, and travel while protecting workers. 6. Protect those most at risk and advance equity, including across racial, ethnic and rural/urban lines. 7. Restore U.S. leadership globally and build better preparedness for future threats. Each of these goals are explained and detailed in the book, with evidence about the current circumstances and how we got here, as well as plans and concrete steps to achieve each goal. Also included is the full text of the many Executive Orders that will be issued by President Biden to achieve each of these goals. The National Strategy for the COVID-19 Response and Pandemic Preparedness is required reading for anyone interested in or concerned about the COVID-19 pandemic and its effects on American society.

The COVID-19 pandemic catalyzed unparalleled speed and cross-sector collaboration in the innovation of tools and technologies to address an emerging infectious disease outbreak. Continued innovation and collaboration in rapid development and implementation of new vaccines, therapeutics, and diagnostics is central to future outbreak preparedness. The National Academies Forum on Microbial Threats; Forum on Drug Discovery, Development, and Translation; and the Forum on Medical and Public Health Preparedness for Emergencies and Disasters held a workshop in December 2022 to explore how innovative approaches in research can enhance health systems preparedness and responses to emerging infectious diseases and dangerous pathogens. Workshop participants reflected on critical scientific infrastructure for stakeholder coordination and innovations that can facilitate rapid and effective preparedness and response to emerging infectious disease threats. This Proceedings of a Workshop summarizes the discussions held during the workshop.
