Increasingly, shoulder surgeries are performed using arthroscopic techniques, leading to reduced tissue damage and shorter patient recovery times. Orthopaedic training programs are responding to the increased demand for arthroscopic surgeries by incorporating arthroscopic skills into their residency curriculums. A need for accessible and effective training tools exists. This thesis describes the design and development of a physical shoulder simulator for training basic arthroscopy skills such as triangulation, orientation, and navigation of the anatomy. The simulator can be used in either the lateral decubitus or beach chair orientation and accommodates wet or dry practice. Sensors embedded in the simulator provide a means to assess performance. A study was conducted to determine the effectiveness of the simulator. Novice subjects improved their performance after practicing with the simulator. A survey completed by experts, recognized the simulator as a valuable tool for training novice surgeons in basic arthroscopic skills.

Few areas in orthopaedics have witnessed the type of exponential growth that has characterized the current field of arthroscopic shoulder surgery. Certainly a debt of gratitude is owed the early pioneers (both open and arthroscopic), whose pas sion and skill fostered today's phenomenon. Rapid advances in imaging, electronic communication, instrumentation, and implant technology, have "suddenly" per mitted arthroscopic access to problems once exclusively the province of the open surgeon. Pioneering work by Lanny Johnson, Dick Caspari, Harv Ellman (and oth ers) further facilitated the application of these emerging technologies into a prac tical and useful new skill set. Today, with few exceptions, most shoulders re quiring surgical intervention can be thoroughly assessed, and in many cases, definitively treated arthroscopically. Yet, and perhaps a consequence of the rather rapid evolution of these tech niques, few single-source references are available for either the "would be" shoul der arthroscopist or the experienced clinician looking to hone his/her arthroscopic skills about the shoulder. Surgical simulation training has been proposed to counter any erosion of surgical education due to limited operating room exposure. Current resident assessment is limited to subjective global rating scales as no standardized models exist to quantifiably assess the improvement in surgical proficiency in the operating room. The question addressed in this study was twofold: 1) Do motion patterns during shoulder arthroscopy correlate with level of expertise, as they do during knee arthroscopy? 2) Can improvement in shoulder arthroscopy technique performed on a cadaveric shoulder model be quantifiably measured?
