In the management of vascular disease, there has been an inexorable drive towards less invasive endovascular treatment options. Endovascular treatment of carotid stenosis for stroke is no exception. Several sizeable multicentre trials are running concurrently, which are comparing carotid endarterectomy (CEA), the "gold standard," with the less invasive alternative, carotid stenting (CAS) for low-risk patients with symptomatic carotid stenoses who are considered to be surgical candidates. The endovascular option is in itself a newcomer but state-of-the-art CAS with regards use of all the available technical refinements, to include low-profile monorail dedicated systems with contemporary pharmacological support, is perhaps all but five years old. There is considerable interest from clinicians from a variety of clinical backgrounds and from industry in this technique and, therefore, scope for a contemporary practical guide.

Over the past three decades, carotid artery stenting has evolved to become a promising and viable alternative to carotid endarterectomy, especially for patients deemed to have high surgical risks. In Carotid Artery Stenting: The Basics, Jacqueline Saw and a panel of international experts on carotid artery stenting discuss in depth the details of all contemporary aspects of carotid stenting, while reviewing supporting studies, guidelines, technical perspectives, and peri-procedural management. This textbook serves as a learning resource on the multifaceted management of patients with carotid artery stenosis, with the key focus on extracranial carotid artery stenting. Additional sections detail the specifics of setting up and maintaining a laboratory and discuss the preparation of the carotid artery stenting procedure from both the patient and operator’s perspective. Authoritative and highly practical, Carotid Artery Stenting: The Basics is an accessible guide and valuable resource for today’s cardiologists, radiologists, and vascular surgeons.

Carotid Angioplasty and Stenting (CAS) is a new approach to treat a carotid stenosis. This new book provides interventional cardiologists, both as beginners or fully experienced, with a reference on all aspects of angioplasty and stenting of the carotid and supra-aortic trunks. Focusing on both the entire range of angioplasty and stenting treatment options for the surgeon treating patients on the operating table, and the range of radiological techniques used for the cardiologist to diagnose carotid artery stenosis (CAS) and associated conditions, this important book describes the best indications, the different techniques, the results, and also the limitations of CAS based on randomized studies and particularly the last published data (CREST study). Suitable for both novice and experienced interventionalists, it also addresses diagnosis of a carotid stenosis and complications from CAS and how to manage them.

Neurointervention is a fast-growing subspecialty, and recent trials have demonstrated its role in ischaemic and haemorrhagic stroke. This has generated tremendous interest among interventional neuroradiology, neurology and neurosurgery communities. Nevertheless, formal teaching programmes that provide the required experience are limited, and many early career practitioners are not exposed to the crucial technical details essential to safely performing the procedure before they start practising independently. The book presents 100 characteristic case studies to illustrate the salient technical and clinical issues in decision-making and problem solving during the procedure. This book conveys the “real-world” issues and solutions that are not addressed in detail in most books. As such it is a practical teaching book with useful “tips and tricks” on how to handle specific challenging situations, and is particularly useful for fellows in neurointervention training programmes..

Operative Dictations in General and Vascular Surgery, Second Edition is intended to teach residents the principles of succinct and precise operative dictations for a wide spectrum of surgical procedures. Designed as a portable resource, the book provides typical dictations to guide the resident. Comprised of 226 procedures, this new edition comprises the majority of procedures commonly performed by general and vascular surgeons. For each procedure, a list of common indications is given. This list covers 95% of the situations in which a particular procedure will be used. A list of essential steps follows. These can be used to mentally rehearse the procedure before it is performed. The next heading, Note these technical variations, introduces a list that is intended to prompt the surgeon for particular things to note and dictate within the template. A list of possible complications that are typically associated with that particular surgical procedure follows. Finally, a template operative dictation is provided. Common opening and terminating sequences (for example, how to enter and close the abdomen) in all laparotomy dictations are included. The volume has two primary objectives. First, it is designed to serve as a source of model operative dictations that may be individualized and used as templates. Second, it is intended as an aide-memoir, giving the surgeon a short list of pertinent information about each procedure. Ample space is provided to allow each surgeon to add notes. By reviewing this introductory material before scrubbing on a procedure, the trainee surgeon will enter the operating room better prepared to participate actively and to learn as much as possible. At the end of training, this book, with the notes accumulated by the resident, will serve as an invaluable review resource analogous to the individualized notebooks many surgeons keep. Operative Dictations in General and Vascular Surgery, Second Edition is intended for practicing surgeons, who may modify each template to reflect their own individual practice. It also serves as a concise reminder of essential steps in those operations that may be only rarely performed.
