Was suddenly seized with most alarming symptoms Of suffocation, constriction and pain within the chest, particularly on the left side, attended with great distress, countenance and skin livid, dyspnoea, and violent paroxysms Of coughing, which ended in her coughing up a quantity of what ultimately proved to be fibrinous tube casts Of the bronchi, equal in bulk to about a large table spoonful, when washed from the accompanying catarrhal expectoration. The sputa were at first, and only at first, very slightly tinged with blood. And a few blood points were Observed on some of the tube casts. In a few hours another paroxysm Of coughing ended in more expectoration of tube casts, the disappearance Of cyanosis, and most marked relief to the patient. Excerpt from Canada Medical and Surgical Journal, Vol. 4: May, 1876 There is very seldom any appearance of blood with the evacuation, but this has occasionally been observed in small quantities. The stools are generally Of moderate consist ence, and come away in small irregular pieces, Often flat tened. On making a digital examination it was found that the rectum was involved in the whole Of its calibre by a dense cartilaginous, irregular mass. The Obstruction began about one inch within the anus, beyond which lay two distinct strictures, through both Of which the finger could be just passed. At the extremity of this again and just as far as the finger could well reach, was felt another but smaller opening through which the finger could not pass at all. The examination gave a great deal of pain. A no. 2 Rectal bougie was subsequently passed, and was found to be just the size Of the uppermost constriction. Excerpt from Canada Medical and Surgical Journal, 1877, Vol. 5: Monthly Record of Medical and Surgical Science Accepting this view of the question, we can suppose a man born one hundred years ago with an insane neurosis, there could, at the present time, be two hundred people the offspring of that man, each of them having the hereditary taint, here there would be a fearful increase of the remote cause of insanity. But when we come to consider how many men existed one hun dred years ago, with an insane neurosis, we have to come to the fearful conclusion that there are but few in the present day, who have not in them the hereditary insane neurosis. I say it is a fearful conclusion to come to, nevertheless if it be true it is better we should recognize it, and guard, as far as it is possi ble, against the exciting cause, which develops the latent malady. In every hereditary disease there must be an exciting cause to develop the malady, a man may be the subject of latent phthisis, and never know it, not even his medical man may suspect it, till he gets an attack of bronchitis or pneumonia, when phthisis will develop itself. A man may be the subject of gout, and never know it till some ordinary disease reduces his system below par, and gout, for the first time, develops itself. A man may have latent cancer in his system, and never know it, till some of his glands receive a physical injury, when the cancer will develop itself. So likewise, a man may have in him an insane neurosis, and neither himself, no any one else, be the wiser of it, till it is developed by its own peculiar exciting cause, and my theory is that the exciting cause that develops insanity is su ering, and I call suffering anything, no matter what it is, even though pleasurable to the person, like excessive venery, that diminishes vital power, or nervous force, and all physical suffering does diminish vital power or nervous force. There was no means of weighing it. The kidneys. Were about normal, &c., in size and appearance, with the exception of being flattened out from pressure, the left one in particular being very much expanded. Some parts of the; cortical substance were paler than others. Some Of the mesenteric glands were a little enlarged Several Of the retro-peritoneal glands were also increased. Its long diameter directed upwards. Not a trace of blood appeared externally. The wound presented the appearance of a packing of 'woollen fibre in blackened and charred tissue, riddled with shot, but so intimately blended as to form one compact mass. I carefully removed all that was removable of the debris; and one very irregularly shaped gpiece of lead aud in the act of vomiting a wad of thread. Excerpt from Canada Medical and Surgical Journal, Vol. 5: October, 1876 Dr. Drake-cut down upon the sciatic nerve at the posterior border of the gluteus maximus muscle, hooked out the nerve, seized it with a strong pair of vulsellum forceps and pulled it forcibly downwards. Before the operation the resp. Were 17, pulse 86, temp. Pupils widely dilated, muscles in a state of permanent spasm. The immediate effect of the nerve stretching was very remarkable. When the nerve was seized with the forceps, the arms and legs were firmly fixed and rigid, the back arched, the head thrown back, and the teeth clenched, although the patient was under chloroform when the first pull was given, the muscular rigidity immediately disappeared, the limbs, which had been tense and hard, became suddenly flaccid, the Opisthotonos relaxed, the jaws could be Opened, and the patient lay quietly and without Spasm on the bed. The pulse and respiration were not visibly affected by the Operation. After being stretched, the nerve was replaced, the wound closed with fine metallic sutures, and carbolized dressing ap plied. The wound in the foot was slit Open with a bistoury, and a few drops of pus let out oiled lint was applied, covered with a hot linseed meal poultice - dressing to be changed every four hours. On coming out of chloroform, the patient felt very roomfortable and drank a quantity of milk with avidity; neither Spasm nor Opisthotonos returning, he sank into a quiet sleep. At 5 pm. The Calabar bean was commenced; 3; grain of the alcoholic extract was given hypodermically every 15 minutes for five hours, then i; grain was given every half hour, so as not to disturb him so frequently. At pm. Slight Spasms returned in the left leg, and he complained of a feeling as if the leg were being violently stretched or pulled all the time; perspiration stood out upon his forehead in beads. Resp. 20, pulse 80, temp. 99° urine was drawn off, causing a severe Spasm with return of slight Opisthotonos the urine flowed tin jets, and was at times propelled a distance of several feet by the sudden contraction of the bladder.
