This report describes the methods and results of the evaluation of the TOP® program as implemented in Hennepin County. The evaluation included two studies: (1) the impact of offering TOP® to middle- and high school-aged youth (the impact study) and (2) the context, implementation fidelity, and challenges faced in implementing the program (the implementation study). Prior to this evaluation, the primary evidence of TOP®'s effectiveness was based on one randomized controlled trial conducted between 1991 and 1995 with 695 teens in 25 high schools across the United States (Allen, Philliber, Herrling & Kuperminc, 1997). The program took place in a mix of in-school and after-school settings, and the youth sample was predominantly female (85%) and African American (67%), with an average age of 15.8 years. The subgroup of adolescent girls participating in the program was significantly less likely than the control group to report a pregnancy during the academic year of the nine-month program; no effects were found for boys (contributing to a pregnancy). The study was not designed to analyze whether this effect was sustained beyond the immediate post-test nor did it include sexual risk-taking behavior outcomes. The study met the HHS Teen Pregnancy Prevention Evidence Review criteria for a high study rating, indicating that it was a well-implemented randomized controlled trial based on the evidence review standards in place in 2010 (Mathematica Policy Research & Child Trends, 2010; Goesling, Colman, Trenholm, Terzian & Moore, 2014).

This report evaluates the Wyman Teen Outreach Program (TOP) using a randomized control trial design. The goal of this evaluation was to replicate the findings of Allen et al. (1997), which demonstrated a significant reduction in problem behaviors, including teen pregnancy, following the TOP® intervention relative to a no-treatment control group within a randomized control trial design. Previous evaluations conducted between 1990 and 2001 demonstrated the effectiveness of TOP® (Allen et al., 1990; Allen et al., 1997; Allen and Philliber, 2001). Further, the Brookings Institution recognized TOP® in its 2007 report to the U.S. Congress, Cost Effective Investments in Children, as one of the best programs in the country, stating that every $1 invested in TOP® produced a $1.29 return on investment (Isaacs, 2007).

The project was implemented in different kinds of settings, with a wide variety of youth (age, ethnicity, gender, language/cultural background, urban and rural). The implementation had a large sample size that allowed for a robust evaluation strategy. This was a large-scale replication of an evidence-based program, TOP®. This program consists of a long-term approach (nine months), high dosage for participants (25 sessions with 20 hours of service learning), a comprehensive curriculum, youth-generated discussion and dialogue, and the development of ongoing relationships with trusted adults. This report describes the implementation and impact of this program including the cluster randomized controlled trial, the implementation process and findings, and the impact study and findings.

Reducing adolescent pregnancy remains a priority for the United States. While rates have been decreasing steadily since the 1990s, important disparities remain, particularly for youth of color and youth living in poverty. Reducing adolescent pregnancy and child-bearing is important for several reasons--childbearing during adolescence negatively affects the parents, their children, and society. Compared with their peers who delay childbearing, adolescent girls who have babies are less likely to finish high school, more likely to rely on public assistance, more likely to be poor as adults, and their children are more likely to have poorer educational, behavioral, and health outcomes over the course of their lives than do children born to older parents (Coyne & D'Onofrio, 2012; Hoffman & Maynard, 2008). There is a clear need to provide effective, evidence-based programs designed to reduce rates of teen pregnancy and unsafe sexual behaviors. The opportunity to provide an intervention that also enhances positive youth development is an additional bonus that benefits adolescents and their communities as a whole.

Florida, the 3rd most populous state in the United States, ranks poorly on most adolescent education and health indicators. Compared with other states, Florida ranked 25th (50th being the worst) in adolescents (16 to 19 years) not going to school and not working ("drop-outs") and 45th in public high school graduation rates in 2009. In 2010, Florida ranked 31st for adolescent (15 to 19 years) live birth rates, 30th for rates of infection with Chlamydia, 34th for rates of infection with Gonorrhea, and 35th for rates of infection with Syphilis. Additionally, Florida had the 3rd highest adolescent (15 to 19 years) HIV diagnosis rate in the nation. One contributor to these poor outcomes is the extent to which Florida's substantial number of non-metropolitan geographic areas are medically underserved. Designed to promote healthy choices, reduce teen pregnancy and increase school success, TOP models an asset-based approach by helping youth develop positive skills, attitudes, and knowledge related to school, problem solving, community engagement, goal setting, and social relationships. This report will contribute to evidence on the program's effectiveness by outlining implementation as well as impact findings (Section IV) and reviewing their implications.

Chicago is a high risk area for teen pregnancies, risky sexual behavior and sexually transmitted infections, with the risk being particularly high in certain neighborhoods. As reported in the 2007 Youth Risk Behavior Survey in Illinois, a higher percentage of ninth-grade students in Chicago have ever had sexual intercourse (51.2%) compared to Illinois (31.1%) or the whole country (32.8%). Additionally, 32% of Chicago youth report not using a condom during their last sexual intercourse, leaving a significant number unprotected from both pregnancy and sexually transmitted infections (STIs). In 2007, the teen birth rate in Chicago was 31.8 per 1,000. While the citywide rate was lower than the national average (42.5 per 1,000), the majority of Chicago communities had higher teen birth rates, and as high as 54.7 per 1,000. Additionally, 96% of teen births in the city are among African-American (57%) and Hispanic (39%) families. Of the 2,424 births to teens under 18 in 2007, 72% occurred in just 28 of Chicago's 77 community areas. Residents in these communities are predominantly low-income and a majority are either African-American or Hispanic. These communities also had some of the highest STI rates among youth under age 18 and accounted for 63% of all chlamydia cases and 65% of all gonorrhea cases in this age group. These communities were the focus of the initiative and evaluation described in this report.

The "Wyman Teen Outreach Program" (TOP) is a life skills curriculum for 12- to 17-year-olds that aims to prevent negative youth behaviors, such as school failure and early pregnancy. Trained facilitators deliver the curriculum in weekly classes throughout the school year. Participants discuss topics such as goal-setting, peer pressure, relationship dynamics, values, and communication skills. The program can be integrated with a school's existing curriculum, offered as an in-school elective, or an after-school program. The What Works Clearinghouse (WWC) identified 9 studies of TOP that were published or released between 1988 and 2008. Seven studies are within the scope of the review and have an eligible design, but do not meet WWC evidence standards, and two studies are out of the scope of the review, as defined by the Dropout Prevention protocol, because they do not present primary research. The lack of studies meeting WWC evidence standards means that, at this time, the WWC is unable to draw any conclusions based on research about the effectiveness or ineffectiveness of TOP.
