Abstract: Severe infections (SI) significantly impact on non-relapse mortality after hematopoietic stem cell transplantation (HSCT). We assessed 432 children and adolescents with acute lymphoblastic leukemia (ALL) after total body irradiation based myeloablative HSCT within the multicenter ALL-BFM-SCT 2003 trial for SI grade 3 or higher according to common terminology criteria for adverse events. A total 172 patients experienced at least one SI. Transplantation from matched unrelated donors (MUD) was associated with any type of SI in the pre-engraftment period (hazard ratio [HR]: 2.57; P

The dogmas of the quiet past are inadequate to the stormy present. The occasion is piled high with difficulty, and we must rise with the occasion. As our case is new, so we must think anew and act anew. Abraham Lincoln, 1862 When I came across this quote, it made me recall my first participation at an international meeting on bone marrow transplantation, at a time when this was the only term that was used to describe the field. During a particular session there was a presentation on the use of peripheral blood as the sole source of stem cells for transplantation, and a member of the audience rose to state that it was medically unethical to consider such treatment, as it certainly could not contain stem cells. Now nearly twenty years later, peripheral blood is the predominant source of stem cells used for hematopoietic stem cell transplantation. In the same period of time there have been several other dogmatic opinions, which permeate all of medicine, that have come and gone in the field of hematopoietic stem cell transplantation, and will continue to do so with advancements from basic and clinical research. It is within this context that the format of this book was devised. Traditionally reviews on specific topics related to hematopoietic stem cell transplantation reflect the views of a single author or a research group.

A comprehensive survey of the current state-of-the-art in hematopoietic stem cell transplantation for malignant disease. The authors focus on the indications and results of transplantation for acute leukemia, chronic myelogenous leukemia, lymphoma, multiple myeloma, and breast cancer. Special attention is given to transplant-related complications, including the pathophysiology and clinical consequences of acute and chronic GVHD, delayed immune reconstitution leading to infectious complications, and organ damage to the lung and liver. Additional chapters address the sources of stem cells and the effects of graft manipulation used to eliminate residual contaminating tumor cells in autologous transplantation, or to reduce the number of T lymphocytes causing GVHD in allogenic transplantation.

Immune Biology of Allogeneic Hematopoietic Stem Cell Transplantation: Models in Discovery and Translation, Second Edition once again provides clinical and scientific researchers with a deep understanding of the current research in this field and the implications for translational practice. By providing an overview of the immune biology of HSCT, an explanation of immune rejection, and detail on antigens and their role in HSCT success, this book embraces biologists and clinicians who need a broad view of the deeply complex processes involved. It then moves on to discuss the immunobiology mechanisms that influence graft-versus-host disease (GVHD), graft-versus-leukemia effect, and transplantation success. Using illustrative figures, highlighting key issues, describing recent successes, and discussing unanswered questions, this book sums up the current state of HSCT to enhance the prospects for the future. The second edition is fully revised and includes new chapters on microbiome, metabolism, kinase targets, micro-RNA and mRNA regulatory mechanisms, signaling pathways in GVHD, innate lymphoid system development, recovery and function in GVHD, genetically engineered T-cell therapies, immune system engagers for GVHD and graft-versus-tumor, and hematopoietic cell transplant for tolerance induction in solid organ grafts. Brings together perspectives from leading laboratories and clinical research groups to highlight advances from bench to the bedside Guides readers through the caveats that must be considered when drawing conclusions from studies with animal models before correlating to clinical allogeneic hematopoietic stem cell transplantation (HSCT) scenarios Categorizes the published advances in various aspects of immune biology of allogeneic HSCT to illustrate opportunities for clinical applications

Recently, the outcomes of patients with acute lymphoblastic leukemia have improved significantly due to the progresses achieved in diagnostics and various therapeutic interventions. In particular, the availability of several novel agents and targeted therapies as well as the provision of safer modalities of stem cell transplants have yielded higher responses and improved survival rates. The role of hematopoietic stem cell transplantation is reviewed in children and adults with acute lymphoblastic leukemia in the era of novel agents and targeted therapies. Various modalities of stem cell therapies in different types of acute lymphoblastic leukemia as well as closely related issues such as graft versus tumor effect, minimal residual disease, and conditioning therapies are discussed thoroughly. In addition, various modalities of novel therapies have been discussed to be efficacious in clinical practice.
