While there have been recent advances in the development of complementary approaches to breast mammography, breast cancer remains a prevalent and devastating disease. For the most part new advances fail to provide real-time, large field of view, high-resolution approaches to breast imaging where both early detection and subsequent management of breast disease are primary targets. The purpose of this three-year study is to evaluate a promising approach to breast imaging using a diffractive energy imaging technology developed by Advanced Diagnostics, Inc. Progress in the first year lead to methods for evaluating the changes to a prototype system and collecting breast imaging data. This report describes progress made during the second year of the study to implement these methods. Reliable and repeatable procedures for quantifying system performance and collecting breast imaging data will be discussed. The results of data collected from a group of individuals with normal breast anatomy, along with preliminary results from simulated breast biopsies will also be presented. These results continue to provide valuable information to the ADI's image quality improvement program and have had a positive impact on system development.

This cross-disciplinary book documents the key research challenges in the mathematical sciences and physics that could enable the economical development of novel biomedical imaging devices. It is hoped that the infusion of new insights from mathematical scientists and physicists will accelerate progress in imaging. Incorporating input from dozens of biomedical researchers who described what they perceived as key open problems of imaging that are amenable to attack by mathematical scientists and physicists, this book introduces the frontiers of biomedical imaging, especially the imaging of dynamic physiological functions, to the educated nonspecialist. Ten imaging modalities are covered, from the well-established (e.g., CAT scanning, MRI) to the more speculative (e.g., electrical and magnetic source imaging). X-ray mammography screening is the current mainstay for early breast cancer detection. It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.

This research project has involved the analysis of a clinical data set of frequency-domain optical mammograms ( -150 patients) to maximize the effectiveness of this breast imaging modality for cancer detection and to guide further improvements of this technique. During the first year, we have computed edge-corrected optical mammograms (N-images) for the whole data set and obtained the corresponding ROC curve. During the second year, we have devised a new approach to tumor oximetry on the basis of spectral data, and we have started developing a second-derivative scheme of image processing. During the third year, we have finalized the second-derivative processing scheme and applied it to the whole set of clinical data. By combining the information provided by the second- derivative optical mammograms at one wavelength, and the oxygenation images computed from data at four wavelengths, we have obtained a new ROC curve that shows a significant improvement upon the ROC curve based on the N-images alone. In summary, we have completed our broad objectives: (1) maximize the clinically useful information extracted from the existing set of optical mammograms; (2) guide further developments of optical mammography by introducing a new approach to tumor oximetry.
