"As there is a growing demand for more time-efficient and effective methods for medical training without putting patients at risk, the role of simulation keeps expanding. Validation of simulators should precede implementation in medical curricula. However, only a small minority of available medical simulators is found to be (appropriately) validated. Researchers, manufacturers and medical trainers are therefore encouraged to collaborate to develop simulators, conduct proper validation studies and select the appropriate simulator. Especially in the field of arthroscopy, a lack of (properly validated) simulators exists. Two high-fidelity arthroscopic knee simulators are studied for face and construct validity, and a pilot study is conducted to develop a solid protocol for a large scale randomized controlled transfer validity study. The acquirement of arthroscopic skills is stimulated by the use of objective tools and metrics to monitor arthroscopic performance and provide feedback. As metrics and complementary thresholds for safety performance during arthroscopy are scarcely studied, maximum allowed forces for safe joint stressing are investigated. Moreover, construct validity of two Global Rating Scales, designed for evaluation of arthroscopic performance is studied. Although the number of (high-fidelity) arthroscopic simulators and performance tracking tools have recently increased, more evidence on their validity and usefulness of simulators and assessment tools is needed. Future studies should focus on high-quality study designs and standardized protocols in order to offer orthopedic residents an adequate training environment to acquire and maintain arthroscopic skills, and to enable performance tracking and proficiency assessment, with the ultimate goal to improve and secure patient safety."--Samenvatting auteur.

The minimally invasive approach of arthroscopy means less pain and faster recovery time for patients compared to open surgery. However, it implies a high difficulty of performance. Therefore, surgeon should remain at a high level of technical and professional expertise to perform such operations. Surgeon's skills are being developed over years of surgical training on animals, cadavers and patients. Nowadays, cadavers and animal specimens present an ethical problem also the practice on real humans is usually risky. For surgeons to reach a high level, new and alternative ways of performing surgical training are required. Virtual reality technology has opened new realms in the practice of medicine. Today, virtual reality simulators have become one of the most important training methods in the medical field. These simulators allow medical students to examine and study organs or any structure of the human body in ways that were not possible few years earlier. Similarly, the surgeon as well as the medical student can gain a valuable experience by performing a particular surgery with an anatomical accuracy and realism as it is actually performed in the real world. Thus, they can practice on virtual operation before they proceed and operate on real patients. In this thesis, a virtual reality training simulator for wrist arthroscopy is introduced. Two main issues are addressed: the 3-D reconstruction process and the 3-D interaction. Based on a sequence of CT images a realistic representation of the wrist joint is obtained suitable for the computer simulation. Two main components of the computer-based system interface are illustrated: the 3-D interaction to guide the surgical instruments and the user interface for haptic feedback. In this context, algorithms that model objects using the convex hull approaches and simulate real time exact collision detection between virtual objects are presented. A force feedback device, coupled with a haptic algorithm, is used as a haptic interface with the computer simulation system. This leads in the development of a low cost system with the same benefits as professional devices. In this regard, the wrist arthroscopy can be simulated and medical students can learn the basic skills required with safety, flexibility and less cost.

Background: Simulators designed for surgical training are a new educational tool that may address the problem of reduced training opportunities for current medical trainees compared to their predecessors. Research has demonstrated performance differences across hand dominance in simulated laparoscopic surgical tasks. To date, no evidence exists with regard to the applicability of this conclusion related to simulated shoulder arthroscopic surgery. The hypothesis of this thesis was that there are no differences in measured outcomes when comparing participant performance during simulated arthroscopic surgical tasks on a right shoulder compared to a left shoulder. Methods: Using the Touch of Life Technologies ArthroSimTM -a virtual reality based arthroscopic simulator - 60 participants were recruited and performed two simulated diagnostic shoulder arthroscopy training modules: one left shoulder while holding the arthroscope in their right hand for the majority of tasks, and one right shoulder while holding the arthroscope in their left hand. Demographics were collected on each participant, in addition to outcome measures of completion score, and time to completion.. Subjective difficulty was assessed using Likert scales. Results: After assessing for normality of data, non-parametric statistical analysis (Wilcoxon Signed Rank Test) was conducted to discern if there was a performance difference amongst the cohort when performing the arthroscopy on the left versus right side. When analyzing subgroups by right or left hand dominance, significant differences existed across a number of the performance variables measured by the simulator. Conclusions: This study demonstrated that there was a significant performance difference when participants performed an arthroscopy on a right or left shoulder - individuals show increased performance doing one shoulder or the other. This data has implications for surgical training curricula in that modified equipment or teaching techniques may be considered based on hand dominance.

This thesis describes the design and development of a physical shoulder simulator for training basic arthroscopy skills such as triangulation, orientation, and navigation of the anatomy. The simulator can be used in either the lateral decubitus or beach chair orientation and accommodates wet or dry practice. Sensors embedded in the simulator provide a means to assess performance. A study was conducted to determine the effectiveness of the simulator. Novice subjects improved their performance after practicing with the simulator. A survey completed by experts, recognized the simulator as a valuable tool for training novice surgeons in basic arthroscopic skills.
