Foundational Knowledge for the Practice of Family Medicine in West Africa is a selection of introductory concepts from literature using literature search and review; a collection of lectures from eminent family physicians from South Africa, Canada, United Kingdom, and United States; lecture notes; seminal essays; and journal articles that guided the author through the transition from general practice to family medicine in the 1980s and 1990s. The concept of family medicine in West Africa was a paradigm shift from a specialty conception of a conglomeration of independent specialties in general practice to a unique specialty with its own distinct identity with a fundamental principle based on the biopsychosocial model. The specialty draws a distinctive identity from its approach to care by placing emphasis on the contextual setting of the patient, starting with the family and extending to the sociocultural and economic environment. This composite approach to health care was new and confusing to the uninitiated medical graduate undergoing a residency training taught in the traditional biomedical reductionist model and to other medical specialists who asked what new knowledge we were bringing to the table that other specialists were not already teaching. As for those teachers of family medicine in the subregion, it was quite clear as to the need for this conceptual framework in family medicine as opposed to the limitation of general practice. This book documents the struggle to position family medicine as a separate specialty of medicine in West Africa.

From a childhood spent playing marbles and climbing trees in Ceylon to a medical career in bustling Singapore, Dr K. Puvanendran’s experiences have been rich and varied. A leading neurologist, he counts kings and presidents among his former patients. This charmingly written autobiography traces the trajectory of his life against the changing landscapes of two vastly different countries. As a boy in Jaffna, Dr Puvanendran found imaginative ways to fill his time. He recounts with great relish the carefree pranks, adventures and school experiences. Woven into the evocation of these simple pleasures are also sobering glimpses of the darker periods in Ceylon’s history. After attending medical college in Colombo, Dr Puvanendran stayed in Ceylon to work before accepting a position at Outram Road General Hospital, now Singapore General Hospital, in 1971. As the narrative unfolds, we read about the roots of his interest in neurology, the highs and lows of his career, the doctors who inspired him and the most memorable medical cases from his fifty years of practice. Some of these intriguing cases include sleep-related crimes, for which he has testified in court as a local pioneer in sleep medicine. Dr Puvanendran’s story takes us through the old world of Ceylon and into the heady post-independence days of Majulah Singapura (“Onward Singapore”, as the national anthem proclaims), offering along the way a successful doctor’s take on the study and practice of medicine.

Current medical teaching neglects the tale the patient tells or fails to tell. I offer evidence suggesting such tales or narratives are central to understanding medicine.
