Nitric Oxide (NO) is a free radical, a gas, and a pluripotent product of mammalian cells. Only a few years ago, scientific discussions of NO were largely held in the context of the chemistry of air pollution. Now, however, the great significance of NO as a signalling and cytotoxic molecule has captured the attention of the biomedical community. This book provides a very up-to-date review of the role of NO in sepsis and ARDS. Consideration is given to NO both as a pathophysiologic mediator as well as a therapeutic agent. An internationally recognized group of scientists and clinicians have contributed chapters dealing with cutting-edge science and practical clinical strategies. Numerous tables and charts have been included to aid the reader in understanding this fascinating and important subject.

At the Sixth Wiggers Bernard Conference, a group of scientists from various disciplines discussed new findings relating to nitric oxide synthase inhibitor in shock, sepsis, and organ failure. Dedicated to the presentation and discussion of both positive and negative findings related to the use of NOS inhibitors, the meeting served as a forum for issues relating to specific and non-specific inhibitors, as well as the role of nitric oxide-oxygen radical interactions. Both experimental and clinical data were presented in the trauma and sepsis field.

Sepsis evolution and organ dysfunction are still an enigmatic topic. Severe infection causes an important aggression of the whole body, and the so-called inflammation mediators play an important role in this evolution. It is however certain that the immune system is a key factor in sepsis progression. The volume focuses on some primary aspects such as the research of the rationale of the immune system activation during sepsis, on the molecular strategies available for ARDS treatment and on techniques of emofiltration and absorption of endotoxins through apheresis in the septic patient.

SEPSIS, A CONDITION that significantly affects the out come of severely injured patients, is characterized by a systemic inflammatory response that is mediated by various cytokines and activated leukocytes.' Pulmonary dysfunction as indexed by pulmonary arterial hyper- tension, decreasing compliance, and VA/Q mismatching leading to hypoxemia is a common sequeis of sepsis. The exact mechanism by which sepsis affects the pulmonary system is unknown. Activation of both leukocytes and endothelial cells in concert with the release of various compounds results in pulmonary vasoconstriction and increased vascular permeability leading to pulmonary failure, which often necessitates ventilatory support. 3 Inhaled nitric oxide (NO) has been reported to act as a selective pulmonary vasodilator without causing systemic vasodilation. This effect has been shown in various animal models of pulmonary arterial hypertension including those induced by thromboxane analogues, hypoxemia, and endotoxemia. The hypoxemia that accompanies endotoxemia has also been improved by NO. 5 These beneficial effects of NO have been documented in patients with chronic obstructive pulmonary disease, adult respiratory distress syndrome (ARDS), congenital heart failure, and pulmonary arterial hypertension. Literally thousands of papers have been published on nitric oxide over the past ten years. But there is no single monograph available that has previously attempted to summarize the important features of the roles of nitric oxide in inflammation. The voluminous literature regarding the incredible range of chemical and biological effects of nitric oxide and reactive nitrogen oxide species, RNOS, may present a tangle of confusing information to the researcher. This volume brings together experts from nitric oxide and inflammation research and presents a concise up-to-date overview as well as future aspects of this rapidly growing field.
