Across the globe, both in developed and developing countries, the population is rapidly ageing. In the fields of sexual and relationship therapy and sexual health, ageing has not been an issue of priority. Too often, ageing is thought of as a process that relates to problems, deficits, and taboos, and less to pleasure, change, growth and diversity. It is treated as a separate life stage and not a process throughout the lifecycle. Healthcare provider practices around older adult sexuality are increasingly the focus of scholarship. Researchers use available scales to study health worker knowledge and behavior in their research. This dissertation argues the need for a new scale, because the available measures are dated and do not account for changes in attitudes and medical advances over three decades. After a review of the increasing role of physicians, social workers, and other health workers providing care for sexual issues throughout history, I describe available scales for measuring knowledge and behaviors about older adult sexuality and review research that employs them. The purpose of the dissertation was to develop a contemporary measure regarding older adult sexuality practices among health workers. I utilized a two-phase plan following Bowen and Guo's 12-step mixed-method approach for scale development (2012). Phase I included a literature review and qualitative interviews with experts on older adult sexuality regarding the construct. Then I created a universe of items, reviewed them with the experts, and revised items. In Phase II, I tested the items with 155 healthcare providers and conducted analysis for reliability and validity. Based on the analysis, I culled items to create a shortened scale. I proposed a scale consisting of 52 items with three subscales: a 25-item Knowledge subscale, a 13-item Attitude subscale, and a 14-item Behavior subscale. The Knowledge subscale showed poor internal reliability (KR-20 = .625). The Attitudes (alpha = .825), and Behavior (alpha = .837) subscales showed good internal reliability. I also evaluated content validity and criterion-related validity for the subscales. Flaws in the methods and analysis make the 52-item proposed scale conceptually unsound. Limitations in generating the pool of items, evaluating the items, and testing reliability and validity did not produce a viable scale. I analyze problems with the methodological approach and propose a redesign that corrects for flaws in the approach employed here. I will use an inductive, social justice model that expands scale development to include interviews with practitioners and older adults. I conclude with a number of research, practice, and policy implications that will result from a redesigned scale.

This book provides a comprehensive overview for those interested in research and promoting sexual health in older people, as well as a “go-to” guide on the topic of sexual behaviour in older adults. Sexual activity is an essential part of intimate relationships, though it tends to decline in frequency as people grow older. Nevertheless, for many older people, sex still remains an important part of their life. There’s a common misconception that as people age, they lose their interest in sex and capacity for sexual behaviour. This is not the case, as a number of national surveys show. The new research presented here indicates that the less sex older people have, the more likely they are to experience mental and physical health problems. Men and women who reported a decrease in the frequency of sexual activities were also more likely to report a deterioration in self-perceived health. Additionally, men with erectile dysfunction were also more likely to be diagnosed with cancer or coronary heart disease. This research has also determined that older adults who experience a decline in sexual activity report poorer well-being than those who maintain their levels of sexual desire, activity and function in later life, and that men who remain sexually active in later life continue to have better cognitive performance compared to those who don’t. Despite these health benefits, medical professionals do little to promote sexual activity in older adults, and the literature suggests that there is a lack of knowledge on how to approach the subject and how to promote such activities. This is the first comprehensive book specifically exploring all areas of sexual behaviour and health in older adults, and drawing on the latest research in this area. It offers a valuable resource for researchers and clinicians in various health fields (medicine, nursing, occupational therapy, psychology, public health, etc.), as well as students in undergraduate and graduate programs.

This literature review was undertaken in order to determine what caregivers needed to know about elderly sexuality, to determine the needs of elderly people related to their sexuality, and to determine how caregivers could best assist them in meeting those needs. In selecting materials to be annotated for this book, the first priority was given to empirical studies. The two major purposes of this study were to gather data relative to the knowledge and attitudes toward sexual expression among sixty-five to seventy-four year olds and to determine whether or not significant differences existed between the sexes. The study focused on six research questions: These were: 1. Do older females and males differ significantly in terms of overall knowledge related to sexual expression? 2. Do older females and males differ significantly in terms of specific knowledge categories related to sexual expression? 3. Do older females and males differ significantly in terms of overall attitudes related to sexual expression? 4. Do older females and males differ significantly in terms of specific attitude categories related to sexual expression? 5. Are there significant differences between older females and males in terms of overall knowledge when they are grouped according to: marital status, education, religious attendance, self perceived health status and occupation? 6. Are there significant differences between older females and males in terms of overall attitudes when they are grouped according to: marital status, education, religious attendance, self perceived health status and occupation? The population selected for study was a voluntary organization of older adults, who resided in Marion County, Oregon. Thirty-two members, ages sixty-five to seventy-four who volunteered to participate in this study comprised the sample. A structured interview guide was utilized for data collection. Validity of the instrument was established through input from professional experts. Reliability was determined by comparing consistency of responses between three groups: a pilot test group, one in-study group matched to the pilot group and one in-study group selected at random. Both parametric and non-parametric statistical tools were used to analyze the data. These included the t test for research question one; Mann Whitney U for research questions two through four; the f test for research question five; and Chi Square for research question six. Although some minor differences were found to exist between males and females in terms of knowledge and attitudes regarding sexual expression, the differences were not found to be significant at the.05 level. Results of the study indicated the existence of misinformation, conflicting attitudes, and the desire for sexuality information by older adults. Because the sample in this study was found to be non-representative of the general older population, results may not be generalizable. Summaries of personal characteristics and self perception can be located in Tables 7 and 8. Suggestions for future replications of the study are made. Recommendations and implications for educators and service providers who work with older adults are also included.
