Objective: To assess facility-level integration of family planning (FP) into HIV care and contraceptive use among HIV-infected women on antiretroviral therapy (ART). Study design: We conducted a national cross-sectional study with key informants and HIV-infected women on ART to assess integration of FP into HIV care at 108 HIV care and treatment centers with >1000 ART clients per year. Separate surveys were administered to key informants and HIV-infected women. We classified facilities offering FP services in the same facility as care as integrated facilities. Results: Overall, 108 facilities throughout Kenya were evaluated, and 4748 HIV-infected women were enrolled. The majority (73%) of facilities offered integrated FP services. Integrated facilities were more common in the Nyanza region than Nairobi (77% vs 35% respectively, p=0.06). Integrated FP services were more likely to be offered in public compared to private facilities [PR: 1.86, 95% CI: 1.11-3.11; p=0.02]. Prevention of mother-to-child transmission of HIV has been a key pillar of global HIV/AIDS prevention strategies. Despite these efforts, there are approximately 220,000 children born with HIV annually (UNAIDS, 2016). Many prevention of mother-to-child transmission of HIV programs focus on reducing transmission during pregnancy, birth, and breastfeeding with anti-retroviral therapy, but these programs are not reaching everyone. Over the past decade, more attention has been paid to reducing horizontal transmission of HIV by reducing the number of unintended pregnancies by HIV-positive women. More specifically, health programs are looking to integrate HIV and family planning services to efficiently provide targeted services to women living with HIV. Yet many low-income countries with a high HIV-burden are often facing weak health-systems, including a shortage in human resources, making the provision of integrated HIV-Family Planning services difficult. As countries begin to task-shift contraceptive services in general, community health volunteers could play an important role in providing HIV-positive women with information and services related to family planning.

In 2011, heads of state and government at the United Nations General Assembly formally acknowledged that NCDs are one of the major challenges to development and committed to integrate NCD prevention and control into other programmes, such as for HIV and broader maternal and child health programmes, especially in PHC. These consolidated guidelines on HIV prevention, testing, treatment, service delivery and monitoring bring together existing and new clinical and programmatic recommendations across different ages, populations and settings, bringing together all relevant WHO guidance on HIV produced since 2016. It serves as an update to the previous edition of the consolidated guidelines on HIV. These guidelines continue to be structured along the continuum of HIV care. Information on new combination prevention approaches, HIV testing, ARV regimens and treatment monitoring are included. There is a new chapter on advanced HIV disease that integrates updated guidance on the management of important HIV comorbidities, including cryptococcal disease, histoplasmosis and tuberculosis. The chapter on general HIV care, contains a new section on palliative care and pain management, and up to date information on treatment of several neglected tropical diseases, such as visceral leishmaniasis and Buruli ulcer. New recommendations for screening and treating of cervical pre-cancer lesions in women living with HIV are also addressed in this chapter. Guidance on service delivery was expanded to help the implementation and strengthening the HIV care cascade. Importantly, this guidance emphasizes the need for differentiated approaches to care for people who are established on ART, such as reduced frequency of clinic visits, use of multi-month drug dispensing and implementation of community ART distribution. The adoption of these efficiencies is essential to improve the quality of care of people receiving treatment and reduce the burden on health facilities, particularly in resource limited settings.

The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.
