Rapporten består av två delar. Per Carlsson: Swedish assessment of extracorporeal shock-wave lithotripsy and alternative technologies for kidney stone treatment. Syftet med uppsatsen är att presentera uppläggningen av en studie för utvärdering av extrakorporal stötvågslitotriptor, ESVL, och att diskutera metodologiska problem. Några preliminära resultat presenteras. Bengt Jönsson: Intertemporal and interregional variations in kidney stone treatment in Sweden. Författaren redovisar regionala skillnader och utvecklingstrender i Sverige när det gäller behandlingsmetoder för njursten.

The use of and interest in extracorporeal shock wave lithotripsy (ESWL) as treatment for renal and biliary stones has grown during recent years, and scientific reports and conferences on the subject are escalating. Nevertheless the knowledge about total consequences of ESWL for the health care sector and society is limited. Contributions by different scientific disciplines are of value to highlight the pros and cons of new medical technology. In this book on the ESWL technology clinical researchers, social scientists and technicians from a number of countries report their experiences of ESWL. the last section is a discussion on the impact of ESWL in the health care setting, its policy implications, and the need for more assessment studies on this technology. The book consists of the documentation of a conference on ESWL arranged by the Center for Medical Technology Assessment and the Department of Urology, Linkoeping University in collaborating with the Swedish Urological Society and the World Health Organization in November 1988.

Since the introduction of extracorporeal shock wave lithotripsy (ESWL) for treatment of kidney stones 1980 the technology has developed greatly and has been applied in new areas such as the treatment of uteral stones, biliary, pancreatic, and salivary stones. To understand the variety of treatment alternatives it is important to be aware of the fact that there are a great number of different ESWL systems with different ways of creating the shock waves (spark-gab, piezoelectric, and electromagnetic). In this paper we will pay attention to a few issues of current interest for extracorporeal shock wave treatment of renal and biliary stones. We have had to simplify and limit some of the descriptions of the situation in Sweden and our own experiences of ESWL treatment in Linkoeping. A brief summary of early results of ESWL for biliary stones is included. Economic and organizational consequences of ESWL are taken up. An attempt is made to assess the spread of ESWL in Sweden. Finally, some proposals for further assessment studies and research are outlined. After ten years ̀use, ESWL has become a standard procedure for treatment of concrements in the urinary tract, and has largely replaced both open and percutaneous surgery. There are, however, differences in the way new ways of managing renal stones have spread in different countries and the use of ESWL varies considerably.

Examines the management of health care technology in 8 countries: Australia, Canada, France, Germany, the Netherlands, Sweden, the U.K & the U.S. Six technologies (or sets of technologies) -- including evaluation & management efforts & how the technologies diffused -- are presented & compared: treatments for coronary artery disease, imaging technologies (CT & MRI scanning), laparoscopic surgery, treatments for end-stage renal disease, neonatal intensive care, & breast cancer screening. Extensive bibliography for each country. Charts & tables.

For the first time, a single reference identifies medical technology assessment programs. A valuable guide to the field, this directory contains more than 60 profiles of programs that conduct and report on medical technology assessments. Each profile includes a listing of report citations for that program, and all the reports are indexed under major subject headings. Also included is a cross-listing of technology assessment report citations arranged by type of technology headings, brief descriptions of approximately 70 information sources of potential interest to technology assessors, and addresses and descriptions of 70 organizations with memberships, activities, publications, and other functions relevant to the medical technology assessment community.

This background paper is part of a larger study on International Differences in Health Care Technology and Spending, which consists of a series of back- ground papers. International Health Statistics: What the numbers mean for the United States was published in November 1993, and International Comparisons of Administrative Costs in Health Care appeared in September 1994. An additional background paper will report on lessons for the United States from a comparison; of hospital financing and spending in seven countries.
